Abstract

PT X merupakan industri yang bergerak pada bidang pembuatan liquid atau cairan yang bernama Dioctyl Phthalate (DOP) dan Diisononyl Phthalate (DINP) yang merupakan bahan baku plastik atau PVC. Saat ini PT X dalam penerapan manajemen risikonya menggunakan ISO 14000, dan ISO 9000 yang mana framework ini dirasa masih kurang sesuai bagi PT X dikarenakan meskipun telah menerapkan mitigasinya, masih terdapat tingkat risiko yang tinggi dalam hasil skoringnya. Penelitian ini dilakukan dengan menggunakan metode House of Risk (HOR) dan Fuzzy Logic. House of Risk digunakan untuk menentukan peringkat risiko beserta mitigasinya, sedangkan fuzzy logic digunakan dalam perhitungan severity dan occurrence. Didapatkan 22 kejadian risiko yang berpengaruh dalam proses supply chain perusahaan. Identifikasi kejadian risiko ini terbagi pada masing-masing proses bisnis yaitu 6 kejadian risiko di proses plan, 4 kejadian risiko di proses source, 10 kejadian risiko di proses make, dan 2 kejadian risiko di proses deliver. Selain itu terdapat 53 agen risiko, yaitu 12 agen risiko di proses plan, 9 agen risiko di proses source, 44 agen risiko di proses make, dan 6 agen risiko di proses deliver. Setelah dilakukan perhitungan FARP dan dilakukan perankingan, didapatkan 3 agen risiko prioritas yaitu A9 (perubahan jadwal produksi supplier), A44 (target produksi tidak tercapai), dan A5 (persediaan bahan baku kurang). Pada HOR 2 diberikan 8 usulan strategi mitigasi. Berdasarkan diagram pareto, strategi mitigasi yang menjadi prioritas yaitu P1 (melakukan perhitungan re order point).

PT X is an industry engaged in the manufacture of liquids called Dioctyl Phthalate (DOP) and Diisononyl Phthalate (DINP) which are plastic or PVC raw materials. Currently, PT X in implementing its risk management uses ISO 14000 and ISO 9000, which this framework is still not suitable for PT X because even though it has implemented its mitigation, there is still a high level of risk in the scoring results. This research was conducted using the House of Risk (HOR) and Fuzzy Logic methods. The House of Risk is used to determine the risk rating and its mitigation, while the fuzzy logic is used to calculate the severity and occurrence. There are 22 risk events that affected the company's supply chain process. The identification of risk events is divided into each business process, 6 risk events in the plan process, 4 risk events in the source process, 10 risk events in the make process, and 2 risk events in the deliver process. Also, there are 53 risk agents, 12 risk agents in the plan process, 9 risk agents in the source process, 44 risk agents in the make process, and 6 risk agents in the deliver process. After calculating FARP and ranking, 3 priority risk agents are obtained, namely A9 (change in supplier production schedule), A44 (production target not achieved), and A5 (insufficient raw material supply). In HOR 2, 8 proposals for mitigation strategies were given. Based on the Pareto diagram, the priority mitigation strategy is P1 (calculating reorder points).
