Abstract

Tandem offloading operation is an offshore operation that places a shuttle tanker behind the FPSO for cargo transfer that has a relatively high collision frequency and collision hazard that can have severe consequences with high operating costs. Hazard identification using the FMEA method obtained the results of failure effects on the system which were predicted on the DP system and propulsion system. Frequency analysis was conducted using the Fault Tree Analysis (FTA) method on Powered Forward Movement (PFM) or drive-off resulting in a frequency value of 5.70999x10-3 while the Event Tree Analysis (ETA) method resulted in a collision frequency value of 3.37x10-3. Consequence analysis using ANSYS at a speed variation of 0.15 m/s results in maximum total deformation is 9.8307 m and maximum equivalent stress is 1.0006x108 Pa; At a speed variation of 0.25 m/s results in maximum total deformation is 9.8307 m and maximum equivalent stress is 1.00796x108 Pa; At a speed variation of 0.7 m/s results in maximum total deformation is 9.8275 m and maximum equivalent stress is 8.5451x107 Pa. Collision with a speed of 0.15 m/s is at risk index 4, which is a small and minor possibility which is at the acceptable acceptance criteria; Collision with a speed of 0.25 m/s is at risk index 4, which is a small and minor possibility which is at the acceptable acceptance criteria; While collision with a speed of 0.7 m/s is at risk index 4, which is a small and minor possibility which is at the acceptable acceptance criteria, therefore risk control does not need to be carried out.
