Abstract

Stroke merupakan salah satu penyakit yang berkontribusi dalam penurunan fungsi kerja motorik, sehingga dapat menimbulkan kelumpuhan pada upper limb maupun lower limb, yang berpengaruh pada performa pasien dalam beraktivitas sehari–hari, sehingga diperlukan adanya penanganan rehabilitasi sesegera mungkin, salah satunya dengan menggunakan stimulus elektrik atau Functional Electrical Stimulation (FES) dan gerakan berulang. Selain metode rehabilitasi yang digunakan, keterlibatan pasien secara aktif serta kondisi fisik dan psikologis juga memengaruhi tingkat keefektifan sistem rehabilitasi. Pasien yang menjalani rehabilitasi gerakan berulang dalam waktu lama bisa menjadi bosan dalam menyelesaikan proses rehabilitasi. Oleh karena itu, diusulkan penggunaan rehabilitasi FES dengan penambahan game interaktif sederhana yang dapat membantu rehabilitasi terasa lebih menyenangkan. Besarnya stimulus elektrik yang diberikan pada tangan impaired didasarkan pada besar kecilnya nilai error selisih bacaan tegangan flex sensor dari tangan normal dengan tangan impaired. Stimulus FES yang diberikan antara 0-65V dengan frekuensi 10-20KHz dan duty cycle 15% dapat diterima oleh subjek tanpa menimbulkan rasa sakit berlebih. Dari hasil penelitian didapatkan bahwa dibandingkan sistem kontrol PID, sistem kontrol fuzzy-PID bisa memperoleh nilai rise-time lebih cepat dan range of motion rata-rata 90,95o yang mendekati nilai sudut fleksi maksimal sudut Proximal Interphalangeal (PIP) yaitu 100o-110o. Pengujian sistem rehabilitasi dengan game interaktif yang dilakukan dengan subjek normal ini memperoleh rata-rata tingkat akurasi 83,3% pada level permainan berarsitektur lambat dan sederhana. Hasil tersebut dapat menandakan bahwa sistem rehabilitasi dengan game interaktif dapat digunakan untuk membantu pemulihan gerakan menggenggam pada jari-jari tangan. Selain itu, hasil kuesioner yang dikumpulkan dari subjek normal menunjukkan bahwa sistem ini menarik dan mereka merasa termotivasi untuk menyelesaikan task-task rehabilitasi. Hasil pengujian dengan subjek normal ini menjadi bahan analisis terlebih dahulu sebelum kemudian dikembangkan untuk memvalidasi keefektifan sistem rehabilitasi ini terhadap subjek pasien pasca stroke secara langsung.

Stroke is a disease that contributes to a decrease in motoric function, so that it can cause paralysis in the upper limb and lower limb, which affects the patient's performance in daily activities, so that rehabilitation treatment is needed as soon as possible, one of which is by using a Functional Electrical Stimulation (FES) and repetitive movements. In addition to the rehabilitation method used, the patient's active involvement, physical, and psychological conditions also affect the effectiveness of the rehabilitation system. Patients who undergo repetitive movements rehabilitation for a long time may become bored in completing the rehabilitation process. Therefore, it is proposed to use FES rehabilitation with the addition of simple interactive games that can help make rehabilitation more enjoyable. The magnitude of the electrical stimulus given to the impaired hand is based on the size of the error value of the difference in the flex sensor voltage readings from the normal hand to the impaired hand. The FES stimulus given between 0-65V with frequency of 10-20KHz and 15% duty cycle can be received by the subject without causing excessive pain. From the results of the study it was found that compared to the PID control system, the fuzzy-PID control system can obtain faster rise-time values and an average range of motion of 90.95o which is close to the maximum flexion angle value of the Proximal Interphalangeal (PIP) angle in the range of 100o -110o . Testing the rehabilitation system using interactive games with normal subjects obtained an average accuracy rate of 83.3% at the game level with slow and simple architecture. These results indicate that a rehabilitation system with interactive games can be used to help restore grasping movements in the fingers. In addition, the results of questionnaires collected from normal subjects indicated that the system was interesting, they felt motivated to complete rehabilitation tasks as well. The results of testing with normal subjects were used as initial analysis before being developed to directly validate the effectiveness of this rehabilitation system on post-stroke patient subjects.
