Abstract

Dalam tugas akhir ini dilakukan analisis resiko kegagalan operasi mooring system SPM (Single Point Mooring) #3 PT Pertamina Hulu Energi ONWJ yang bertujuan untuk mengetahui apa saja risiko kegagalan operasi yang terdapat pada mooring system SPM (Single Point Mooring) #3 serta frekuensi dan keparahan kegagalannya dengan menggunakan metode HAZOP dan Bowtie Analysis. Tujuan dilakukannya tugas akhir ini adalah untuk mengetahui pengendalian terhadap dampak risiko yang dapat terjadi serta membahas konsekuensi kegagalannya. Penelitian ini dimulai dengan melakukan identifikasi hazard lalu dilanjutkan dengan penilaian risiko, kemudian menentukan langkah-langkah pengendalian potensi bahaya dominan yang ada, hingga pada akhirnya memvisualisasikan risiko dominan ke dalam diagram BowTie. Dalam penelitian ini dilakukan penyebaran kuisioner kepada pihak terkait untuk mengetahui besaran frekuensi (frequency) dan keparahan (severity) potential hazard pada mooring system SPM (Single Point Mooring) #3. Dari hasil penelitian ini didapatkan bahwa potensi bahaya pada mooring system SPM (Single Point Mooring) #3 pada saat kondisi sedang beroperasi yang termasuk kategori high risk adalah mooring hawser broke pada aktivitas Mooring hawser connection (FSO AA – SPM#3), collision with FSO AA pada aktivitas Crude Oil Transfer, dan winch failure pada aktivitas Mooring hawser disconnection (FSO AA – SPM #3).

In this study, an analysis of the risk of operational failure of the SPM (Single Point Mooring) #3 mooring system was carried out at PT Pertamina Hulu Energi ONWJ, which aims to determine the risk of operating failures contained in the SPM (Single Point Mooring) #3 mooring system and the probability of its failure. using the HAZOP and Bowtie Analysis methods. This research aims to determine the control of the impact of risks that can occur and discuss the consequences of failure, which are determined based on their impact on safety, the environment, and business. This research begins by identifying the hazard and then proceeds with a risk assessment, then determines the existing dominant risk control measures, and finally visualizes the dominant risk into a BowTie diagram. In this study, questionnaires were distributed to related parties to determine the frequency and severity of potential hazards in the SPM (Single Point Mooring) #3 mooring system. From the results of this study, it was found that the potential dangers of the SPM (Single Point Mooring) #3 mooring system when operating conditions were included in the high-risk category were mooring hawser broke in mooring hawser connection (FSO AA – SPM#3) activity, collision with FSO AA on crude oil transfer activity, and winch failure on Mooring hawser disconnection activity (FSO AA – SPM #3).
