Abstract

Dalam proses produksi masal produk Ls, PT XYZ telah mengeluarkan biaya scrap yang tinggi karena tidak tercapainya kualitas yang memenuhi spesifikasi pelanggan. Tinggi nya persentase reject produk juga dapat memberikan potensi terkirimnya produk reject kepada pelanggan, yang dapat merugikan perusahaan berupa cancelling order dalam beberapa periode oleh pelanggan senilai $40.000 setiap bulannya. Oleh karena itu, tujuan dari penelitian ini adalah untuk menganalisis potensi akar permasalahan pada proses produksi yang menyebabkan tingginya persentase reject pada produk Ls. Metode yang digunakan adalah Process Failure Mode and Effect Analysis (PFMEA), dengan mengolah nilai occurrence, severity, dan detection dari setiap mode kegagalan. Dengan PFMEA, diketahui nilai RPN yang menunjukan potensi risiko tertinggi dari identifikasi mode kegagalan. Metode Pareto dan Logic Tree Analysis (LTA) kemudian digunakan untuk mengidentifikasi prioritas potensi kegagalan menjadi beberapa kategori, yaitu safety, outage, minor case, dan hidden case. Berdasarkan hasil penelitian, proses stamping merupakan proses yang memberikan potensi sebesar 50% terhadap reject produk. Dua mode kegagalan pada proses stamping berisiko pada aspek safety operator. Oleh karena itu, rumusan mitigasi diajukan untuk mencegah terjadinya risiko tersebut baik secara langsung dan untuk jangka panjang, berupa corrective dan preventive action. Corrective action dilakukan dengan melakukan pengecekan atau perbaikan secara langsung dan rencana pengecekan berkala terhadap performa peralatan, dan penambahan unit pendukung perlatan. Preventive action dilakukan dengan melakukan study terhadap reliabilitas peralatan. Respons yang diharapkan pihak manajerial berupa penyediaan suku cadang tooling, keleluasaan untuk melakukan study reliabilitas tooling, serta diadakannya pelatihan ulang bagi operator berkaitan dengan aspek keselamatan kerja. Dengan demikian, proses produksi akan terhindar dari potensi kegagalan yang memicu kepada reject produk.

In the process of mass production of Ls products, PT XYZ has incurred high scrap costs due to failure to achieve quality that meets customer specifications. A high percentage of rejected products can also create the potential for rejected products to be sent to customers, can cost the company $40,000 each month in customer orders cancellation over several periods. Therefore, the aim of this research is to analyze the potential root problems in the production process which cause a high percentage of rejects in Ls products. The method used is Process Failure Mode and Effect Analysis (PFMEA), by processing the occurrence, severity level and detection of each failure mode. With PFMEA, the RPN value be known which indicates the highest potential risk of identification mode failure. The Pareto method and Logic Tree Analysis (LTA) are then used to identify potential failure priorities into several categories, which are safety, outage, minor cases, and hidden cases. Based on research results, the stamping process is a process that provides a potential of 50% for rejected products. Two failure modes in the stamping process pose a risk to operator safety. Therefore, the mitigation formulation is intended to prevent the occurrence of these risks both immediately and in the long term, in the form of corrective and preventive actions. Corrective action is carried out by carrying out direct checks or repairs and planning periodic checks on equipment performance, and additional equipment support units. Preventive action is taken by conducting a study of equipment reliability. The managerial responses are expected to be in the form of providing tool spare parts, freedom to carry out tool reliability studies, as well as holding retraining for operators related to work safety aspects. In this way, the production process will avoid potential failures that trigger product rejection.
