Abstract
Dari tahun ke tahun demand pupuk semakin meningkat. Oleh karena itu PT Petrokimia berusaha untuk memenuhi demand dengan tetap mengutamakan kualitas produknya. Namun dalam menjalankan proses bisnisnya PT Petrokimia Gresik memiliki risiko yang akan mengganggu kualitas produk. Pada penelitian ini digunakan pendekatan quality risk management untuk mengetahui risiko-risiko apa saja yang mengganggu kualitas pupuk Phonska. Untuk mengidentifikasi dan menganalisa risiko digunakan tool kerangka keija manajemen risiko yaitu AS/NZS 4360:2004. Risiko-risiko yang berpotensi mengganggu kualitas produk antara lain gangguan peralatan screen, gangguan feeding bahan baku, gangguan feeding pigmen wama, produk rusak, perubahan kualitas bahan, ketidaksesuaian berat produk, gangguan peralatan pada dryer. Berdasarkan hasil penilaian risiko, yang menghasilkan nilai Dampak x Peluang paling besar adalah risiko gangguan pada peralatan screen dan yang nilainya rendah adalah risiko gangguan peralatan dryer. Setelah dilakukan evaluasi maka yang tergolong pada high risk adalah risiko gangguan peralatan screen, dan gangguan feeding bahan baku. Kemudian diberikan usulan mitigasi untuk mengendalikan risiko tersebut

From year to year the demand for fertilizer increases. Therefore, PT Petrokimia tries to meet demand while still prioritizing the quality of its products. However, in carrying out its business processes, PT Petrokimia Gresik has risks that will disrupt product quality. In this research, a quality risk management approach was used to find out what risks interfere with the quality of Phonska fertilizer. To identify and analyze risks, the risk management work framework tool, namely AS/NZS 4360:2004, is used. Risks that have the potential to disrupt product quality include screen equipment disruptions, raw material feeding disruptions, colored pigment feeding disruptions, damaged products, changes in material quality, product weight discrepancies, and dryer equipment disruptions. Based on the results of the risk assessment, the one with the greatest Impact x Opportunity value is the risk of disruption to the screen equipment and the one with the lowest value is the risk of disruption to the dryer equipment. After the evaluation is carried out, what is classified as high risk is the risk of screen equipment disruption and raw material feeding disruption. Then mitigation proposals are provided to control these risks
