Abstract
Kecelakaan kerja merupakan hal yang harus dihindari dalam kegiatan industri. Penelitian ini berfokus pada analisis kecelakaan kerja pada area stamping di PT.XYZ. Tujuan penelitian ini yaitu menentukan Hazard Operability dan dilanjutkan dengan memberikan rekomendasi atas bahaya tersebut. Penelitian ini menggunakan metode HAZOP berbasis logika fuzzy dan juga metode FMEA sehingga memberikan hasil yang lebih baik. Data yang digunakan yaitu antara lain data identifikasi K3 perusahaan, data kecelakaan kerja, dan data kerusakan mesin. Analisis hazard operability menggunakan metode HAZOP dan fuzzy ditemukan bahwa 39 risiko bahaya yang mungkin/telah terjadi dengan kategori rendah sebesar 30,77% dan kategori sedang sebesar 69,23% yang mengacu pada AS/NZS 4360:2004. Pada penggunaan metode FMEA didapatkan 5 item utama yang memiliki mode kegagalan/kerusakan mesin paling tinggi dan ditemukan nilai Critically dengan kategori very high sebesar 26,08%, lalu pada kategori high sebesar 60,87%, sedangkan pada kategori medium sebesar 13,04% dari total mode kegagalan yang ditemukan yang mengacu pada penelitian sebelumnya baik dari jurnal,thesis, dan sebagainya. Rekomendasi yang didapatkan diharapkan mampu menghilangkan atau mengurangi bahaya yang ada.

Work accidents are things that must be avoided in industrial activities. This study focuses on the analysis of work accidents in the STAMPING area at PT.XYZ. The purpose of this study is to determine the Hazard Operability and proceed with providing recommendations for these hazards. This study uses the HAZOP method based on fuzzy logic and also the FMEA method so as to provide better results. The data used include the company's K3 identification data, work accident data, and engine damage data. Hazard operability analysis using the HAZOP and fuzzy methods found that 39 possible/have occurred hazards in the low category of 30.77% and the medium category of 69.23% which refers to AS/NZS 4360:2004. Using the FMEA method, there were 5 main items that had the highest engine failure/damage mode and the Critically value was found in the very high category of 26.08%, then in the high category of 60.87%, while in the medium category it was 13.04% of the total failure modes found referring to previous research from journals, theses, and so on. The recommendations obtained are expected to be able to eliminate or reduce existing hazards.
