Abstract
Penilaian risiko keselamatan merupakan aspek penting dalam mengelola sistem yang kompleks, terutama di industri manufaktur yang mengutamakan keselamatan. Kecelakaan kerja merupakan suatu kejadian tak terduga yang dapat mengakibatkan cedera, kerusakan harta benda, kehilangan waktu, dan berpotensi menimbulkan akibat yang fatal. Metode kerja yang buruk, kondisi bangunan, dan berbagai tugas proyek berkontribusi terhadap penyebab kecelakaan tersebut. Untuk memitigasi risiko-risiko ini, penting untuk menerapkan manajemen risiko dalam proyek. Tujuan utama manajemen risiko proyek melibatkan identifikasi, analisis, dan penanganan atau meminimalkan potensi risiko. PT. Dinito Jaya Sakti, perusahaan yang diteliti dalam penelitian ini merupakan industri manufaktur plastik yang beroperasi sehari-hari, perusahaan ini mempekerjakan kurang lebih 600 orang pekerja secara bergilir di 5 departemen utama berdasarkan struktur organisasinya. Setiap departemen, berdasarkan struktur organisasinya, menghadapi lebih dari 12 bahaya berbeda dan risiko terkait, sehingga sulit untuk memprediksi dan mencegah kecelakaan sepenuhnya. Penelitian ini mengeksplorasi penerapan Fuzzy TOPSIS sebagai metodologi untuk meningkatkan kelengkapan dan akurasi penilaian risiko keselamatan. Fuzzy TOPSIS, teknik pengambilan keputusan multi-kriteria, mengakomodasi ketidakpastian dan ambiguitas yang melekat dalam penilaian risiko keselamatan. Studi ini dimulai dengan menetapkan serangkaian kriteria keselamatan yang komprehensif, termasuk faktor kuantitatif dan kualitatif. Angka fuzzy digunakan untuk mewakili ketidakpastian linguistik yang terkait dengan penilaian ahli, sehingga berkontribusi terhadap pemodelan parameter keselamatan yang lebih realistis. Metode Fuzzy TOPSIS kemudian diterapkan untuk menentukan peringkat dan memprioritaskan berbagai risiko keselamatan berdasarkan tingkat keparahan dan potensi dampaknya. Hasil penelitian ditemukan terdapat beberapa faktor risiko dominan yang teridentifikasi dari 5 departemen di PT. Dinito Jaya Sakti. Yaitu 6 faktor risiko pada departemen PPIC, 7 faktor risiko pada departemen moldshop, 7 faktor risiko pada departemen produksi, 6 faktor risiko pada departemen maintenance, dan 5 faktor risiko pada departemen engineering sehingga total terdapat 36 faktor risiko dominan. Oleh karena itu, setelah mengelompokkan risiko dan bahaya berdasarkan 4 jenis bahaya, ditemukan bahwa risiko yang paling krusial sebagian besar disebabkan oleh 2 bahaya, yaitu mesin dan desain ergonomis atau peralatan yang tidak tepat

Safety risk assessment is an important aspect of managing complex systems, especially in manufacturing industries where safety is a priority. A work accident is an unforeseen event that can lead to injuries, damage to property, time loss, and potentially fatal outcomes. Poor work methods, building conditions, and various project tasks contribute to the causes of such accidents. To mitigate these risks, it is essential to implement risk management in projects. The primary goals of project risk management involve identifying, analyzing, and addressing or minimizing potential risks. PT. Dinito Jaya Sakti, the company under investigation in this study is a plastic manufacturing industry operating on a daily basis, the company employs approximately 600 workers in rotating shifts across 5 main departments based on their organizational structure. Each department, as per the organizational structure, faces more than 12 distinct hazards and associated risks, making it challenging to predict and prevent accidents entirely. This research explores the application of the Fuzzy TOPSIS as a methodology to improve the completeness and accuracy of safety risk assessments. Fuzzy TOPSIS, a multi-criteria decision-making technique, accommodates the uncertainty and ambiguity inherent in safety risk assessment. The study began by establishing a comprehensive set of safety criteria, including both quantitative and qualitative factors. Fuzzy numbers are used to represent the linguistic uncertainty associated with expert judgment, thereby contributing to more realistic modeling of safety parameters. The Fuzzy TOPSIS method is then applied to rank and prioritize various safety risks based on their severity and potential impact. The results It was found that there were several dominant risk factors identified from 5 departments at PT. Dinito Jaya Sakti. Namely 6 risk factors in the PPIC department, 7 risk factors in the moldshop department, 7 risk factors in the production department, 6 risk factors in the maintenance department, and 5 risk factors in the engineering department for a total of 36 dominant risk factors. Therefore after grouping the risk and hazard based on 4 types of hazard, it was found that the most crucial risk mostly is caused by 2 hazard, namely machine and ergonomical design or improper equipment
