Abstract

Dalam konteks layanan kesehatan, industri kesehatan merupakan praktik medis yang dinamis dan berkembang dengan cepat. Menurut Peraturan Menteri Kesehatan Republik Indonesia Nomor 16 Tahun 2016, sebuah rumah sakit wajib memiliki manajemen risiko untuk menjauhkan pasien, karyawan, dan lingkungan rumah sakit dari potensi bahaya termasuk ruang rawat inap. Namun, Rumah Sakit XYZ tidak memiliki tim manajemen risiko yang telah terbentuk karena masing-masing dari kepala departemen akan mengola risiko yang teridentifikasi. Selain itu, proses identifikasi potensial risiko yang terjadi di rumah sakit masih perlu dipertegas dan ditangani. Dalam ruang rawat inap di Rumah Sakit XYZ telah mengalami banyak kejadian kurang baik yang seringkali disebabkan oleh kelalaian perawat. Padahal pada umumnya pasien menghabiskan waktu yang lama dalam menerima perawatan di ruang rawat inap. Sehingga membuat peran ruang rawat inap sebagai pusat penting bagi proses perawatan dan pemulihan pasien mengingat sebagian besar perawatan dan intervensi dilakukan dalam ruangan ini. Studi ini akan mengatasi masalah tersebut dengan berfokus pada aktivitas keperawatan di ruang rawat inap. Dimulai dengan mengidentifikasi risiko berdasarkan aktivitas proses bisnis ruang rawat inap dengan menggunakan model Integrated Definition Methods (IDEF0). Selanjutnya, standar ISO 31000:2018 akan digunakan untuk menerapkan praktik manajemen risiko. Serta metodologi Healthcare Failure Mode and Effect Analysis (HFMEA) untuk mengembangkan pendekatan sistematis dalam identifikasi risiko dan rekomendasi tindakan pengelolaan risiko.
Hasil penelitian mengungkapkan total 178 risiko yang teridentifikasi berasal dari 6 aktivitas utama dan 92 sub-aktivitas. Dengan menggunakan metodologi HFMEA, terdapat 64 risiko yang diprioritaskan dari total 534 risiko yang telah dievaluasi. Sehingga terdapat 18 risiko tipe perlakuan terminate, 7 risiko tipe perlakuan transfer, 39 risiko tipe perlakuan treat, dan 230 risiko tipe mitigasi tolerate. Berdasarkan hasil tersebut, sangat banyak risiko dengan tipe perlakuan tolerate karena risiko – risiko tersebut sudah sesuai dengan tingkat risk appetite dan toleransi risiko yang telah ditentukan oleh Rumah Sakit XYZ.

In the complex landscape of healthcare delivery, risk management is essential to safeguard patients, employees, and the hospital environment, as mandated by the Republic of Indonesia Minister of Health Regulation No.16 of 2016. However, Hospital XYZ has no designated risk management team because each department head will process the identified risks. In addition, the process of identifying potential risks that occur in hospitals still needs to be clarified and addressed. Since Hospital XYZ has experienced many adverse incidents in its inpatient rooms, numerous adverse incidents occurred in Hospital XYZ's inpatient rooms, often attributed to nurse negligence. Given that patients typically spend the longest duration receiving treatment in the inpatient room, its significance in the patient's healing process is paramount. This is because the majority of treatment and interventions are conducted within the confines of the inpatient room, making it a crucial hub for patient care and recovery processes. This study addresses these issues by focusing on nursing activities in inpatient rooms. It will commence by identifying risks based on the business process activities of the inpatient rooms, utilizing the Integrated Definition Methods (IDEF0) framework. Subsequently, the ISO 31000:2018 standard employed to implement risk management practices. Additionally, integrating the Healthcare Failure Mode and Effect Analysis (HFMEA) methodology to develop a systematic approach to risk identification and treatment action recommendations. The research result reveals total of 178 identified risks originates from 6 main activities and 92 sub-activities. By incorporating HFMEA methodology, there are 64 prioritized risk out of 534 risks evaluated. Resulting in 18 risk categorized as terminate risk, 7 risks categorized as transfer risks, 39 risks categorized as treat risk, and 230 risks categorized as tolerated. The predominant strategy primarily consists of the tolerate risk category, as it aligns with Hospital XYZ's predetermined risk appetite and tolerance levels.
