Abstract

Setiap Pelabuhan Petikemas memerlukan Fondasi sebagai struktur penguatan Dermaga untuk menahan beban lateral yang disebabkan oleh beban lingkungan seperti Arus,Gelombang dan Gempa serta beban operasi kapal seperti mooring dan berthing. Dalam proses Instalasi tiang pancang ini ada banyak risiko yang dapat terjadi dalam proses tersebut. Dalam penelitian ini tugas akhir ini, dilakukan analisis risiko pada proses instalasi tiang pancang terminal petikemas. Dengan menggunakan metode Fuzzy Failure Mode and Effect Analysis (Fuzzy-Fmea) untuk mengetahui risiko dominan dari setiap risiko melalui proses Deffuzifikasi dengan output Fuzzy RPN (FRPN). Diperoleh 3 risiko dominan tertinggi yaitu Tiang pancang jatuh dan menimpa pekerja dengan nilai FRPN 8.99, Crane mengenai pekerja dengan nilai FRPN 8.63 dan Crane terperosok atau Terguling dengan nilai FRPN 8.06. Hasil Perhitungan FRPN tersebut dilakukan analisa menggunakan Fault Tree Analysis untuk mencari akar penyebab dari ketiga risiko dominan tersebut dari hasil pembuatan pohon kegagalan didapatkan 14 akar penyebab risiko dominan tersebut. Selanjutnya setelah mengetahui risiko dominan dan penyebab nya dilakukan pemberian mitigasi risiko guna mengurangi terjadinya risiko tersebut.

Each Container Terminal requires a foundation as a strengthening structure for the dock to withstand lateral loads caused by environmental factors such as currents, waves, and earthquakes, as well as operational loads from ships like mooring and berthing. During the installation process of these piles, there are numerous risks that can occur. In this final project study, a risk analysis was conducted on the pile installation process at the container terminal. By using the Fuzzy Failure Mode and Effect Analysis (Fuzzy-FMEA) method, the dominant risks of each potential hazard were identified through the defuzzification process with the output being the Fuzzy Risk Priority Number (FRPN). The three highest dominant risks identified were: piles falling and hitting workers with an FRPN value of 8.99, the crane hitting workers with an FRPN value of 8.63, and the crane slipping or overturning with an FRPN value of 8.06. The FRPN calculation results were then analyzed using Fault Tree Analysis to find the root causes of these three dominant risks. From the failure tree creation, 14 root causes of these dominant risks were identified. Subsequently, after identifying the dominant risks and their causes, risk mitigation measures were implemented to reduce the occurrence of these risks.
