Abstract
Loadout adalah proses menarik deck/module ke atas barge untuk ditransportasikan ke site tempat struktur tersebut diinstalasi. Pada proses loadout jacket platform masi banyak terjadi beberapa kecelakaan kerja yang berdampak pada pekerja. Analisis risiko dilakukan untuk mengurangi dampak dan potensi terjadinya risiko kecelakaan kerja yang ditimbulkan dari proses loadout jacket platform. Pada tugas akhir ini akan dilakukan identifikasi dan analisa risiko kecelakaan kerja pada proses loadout jacket platform dengan menggunakan metode analisis Fuzzy Failure Mode and Effect Analysis (FFMEA) yang dapat menunjukkan risiko dominan melalui proses fuzzyfikasi dan menghasilkan fuzzy-RPN (FRPN). Diperoleh risiko dominan tertinggi pada risiko pekerja terjatuh kedalam tanki ballast dengan FRPN 7,920. Selanjutnya dilakukan analisis risiko dengan metode Fault Tree Analysis (FTA) untuk mendapatkan akar-akar masalah dari risiko dominan pada proses loadout jacket platform tersebut yakni kondisi tanki ballast tidak aman dengan probabilitas 0,000897 dan ketidakwaspadaan pekerja dengan probabilitas 0,00198. Setelah mengetahui risiko dominan dan penyebabnya, maka dilakukan pengendalian risiko yang menghasilkan upaya mitigasi untuk meminimalisir terjadinya risiko kecelakaan kerja tersebut.

Loadout is the process of pulling a deck/module onto a barge for transportation to the site where the structure is installed. In the jacket platform loadout process, there are still many work accidents that have an impact on workers. Risk analysis is carried out to reduce the impact and potential risk of work accidents arising from the jacket platform loadout process. In this final project, identification and analysis of work accident risks in the jacket platform loadout process will be carried out using the Fuzzy Failure Mode and Effect Analysis (FFMEA) analysis method which can show the dominant risk through the fuzzyfication process and produce a fuzzy-RPN (FRPN). The highest dominant risk is obtained at the risk of workers falling into the ballast tank with FRPN 7.920. Furthermore, risk analysis is carried out using the Fault Tree Analysis (FTA) method to get the root causes of the dominant risks in the jacket platform loadout process, namely unsafe ballast tank conditions with a probability of 0.000897 and worker unawareness with a probability of 0.00198. After knowing the dominant risk and its causes, risk control is carried out which results in mitigation efforts to minimize the risk of work accidents.
