Abstract
Tumor merupakan pertumbuhan sel abnormal yang dapat bersifat jinak maupun ganas. Jumlah kasus tumor terus meningkat, menciptakan beban kesehatan semakin besar. Prosedur operasi menjadi pendekatan utama dalam penyembuhan pasien penderita tumor. Setelah operasi, pasien memasuki fase pemulihan yang melibatkan perbaikan fisik dan peningkatan kualitas hidup. Selain efek samping fisik, pasien juga menghadapi tantangan sosial, psikologis, dan spiritual. Rumah Sakit Nur Hidayah Bantul telah menerapkan terapi Hu-Care, yang menggabungkan konsep husnul khatimah dan perawatan paliatif, untuk mendukung pemulihan pasien secara holistik. Informasi mengenai waktu pemulihan pasien pascaoperasi tumor dengan terapi Hu-Care sangat penting sebagai bagian dari upaya pengobatan. Penelitian ini menggunakan regresi Cox untuk menganalisis waktu pemulihan pasien pascaoperasi tumor yang menerima terapi Hu-Care di Rumah Sakit Nur Hidayah Bantul. Metode regresi Cox sering digunakan untuk analisis survival, tetapi memiliki keterbatasan dalam mengakomodasi variasi antar pasien yang tidak dapat dijelaskan oleh variabel yang terukur. Oleh karena itu, penelitian ini menggunakan regresi Cox dengan frailty untuk mengatasi keterbatasan tersebut. Semua variabel yang digunakan telah memenuhi asumsi proportional hazard sehingga regresi Cox yang digunakan adalah Cox proportional hazard. Hasil penelitian menunjukkan bahwa 89,5% pasien mengalami pemulihan. Sebagian besar pasien berusia di sekitar 40 tahun, dengan lama operasi sekitar 30 menit, dengan ukuran tumor median 4,8 hingga 5,75 gram, dan kadar hemoglobin praoperasi 10-15 g/dL. Mayoritas pasien adalah perempuan, menjalani operasi ringan pada sistem reproduksi, menggunakan anestesi umum, memiliki ASA I, tanpa riwayat diabetes melitus atau hipertensi, pernah menikah, dan merasa nyaman secara psikospiritual. Median waktu survival pasien adalah 11 hari. Kaplan-Meier dan uji log-rank menunjukkan variabel jenis kelamin, jenis operasi, lokasi tumor yang dioperasi, ASA, jenis anestesi, dan status pernikahan memiliki perbedaan kurva survival antar kategorinya. Model regresi Cox proportional hazard dengan frailty menunjukkan bahwa lokasi tumor yang dioperasi, jenis anestesi, status perkawinan, dan faktor frailty secara signifikan mempengaruhi waktu pemulihan pasien pascaoperasi tumor yang menerima terapi Hu-Care. Terdapat variasi antar pasien sebesar 0,4871 yang tidak dapat dijelaskan oleh variabel yang teramati. Pasien yang berisiko mengalami pemulihan lebih cepat adalah pasien yang menjalani operasi tumor pada sistem integumen, menggunakan anestesi spinal, dan pernah menikah.

Tumors are abnormal cell growths that can be benign or malignant. The number of tumor cases continues to increase, creating a greater health burden. Surgical procedures are the main approach in healing patients with tumors. After surgery, patients enter a recovery phase that involves physical repair and improved quality of life. In addition to physical side effects, patients also face social, psychological and spiritual challenges. Nur Hidayah Bantul Hospital has implemented Hu-Care therapy, which combines the concepts of husnul khatimah and palliative care, to support holistic patient recovery. Information regarding the recovery time of patients after tumor surgery with Hu-Care therapy is very important as part of treatment efforts. This study used Cox regression to analyze the recovery time of postoperative tumor patients who received Hu-Care therapy at Nur Hidayah Bantul Hospital. The Cox regression method is often used for survival analysis but has limitations in accommodating variation between patients that cannot be explained by measured variables. Therefore, this study used Cox regression with frailty to overcome these limitations. All variables used have met the proportional hazard assumption so that the Cox regression used is Cox proportional hazard. The results showed that 89.5% of patients experienced recovery. Most patients were around 40 years old, with a surgery time of about 30 minutes, with a median tumor size of 4.8 to 5.75 grams, and a preoperative hemoglobin level of 10-15 g/dL. Most patients were female, underwent minor surgery on the reproductive system, used general anesthesia, had ASA I, no history of diabetes mellitus or hypertension, were married, and were psychospiritually comfortable. The median survival time was 11 days. Kaplan-Meier and log-rank tests showed the variables of gender, type of surgery, location of the operated tumor, ASA, type of anesthesia, and marital status had different survival curves between categories. Cox proportional hazard regression model with frailty showed that the location of the operated tumor, type of anesthesia, marital status, and frailty factors significantly affected the recovery time of postoperative tumor patients receiving Hu-Care therapy. There was an inter-patient variation of 0.4871 that could not be explained by the observed variables. Patients at risk for faster recovery were those who underwent tumor surgery on the integumentary system, used spinal anesthesia, and were married.
