Abstract

Gagal ginjal kronis merupakan penyakit tidak menular yang melanda lebih dari 10 persen populasi dunia, termasuk negara Indonesia. Penyakit ini, seperti gagal ginjal kronis, umumnya tidak bisa disembuhkan hanya dengan melakukan perawatan dalam jangka waktu yang cukup lama, seperti hemodialisis, yang sering menjadi perawatan jangka panjang. Perawatan medis yang cukup lama ini dapat menyebabkan kecemasan atau depresi pada pasien, sehingga beberapa tahun terakhir telah dikembangkan terapi komplementer yang menitikberatkan aspek psikologis dan spiritual. Di Indonesia, telah dikembangkan praktik Hu-Care untuk mendukung proses penyembuhan pasien kronis. Hu-Care diharapkan dapat memberikan pengaruh positif pada pasien sehingga membantu dalam proses penyembuhan. Penelitian ini menggunakan regresi Cox untuk dapat mengetahui pengaruh psikospiritual terhadap ketahanan hidup pasien hemodialisis dan melakukan diagnostic checking pada model regresi Cox. Data yang digunakan merupakan data sekunder dari rekam medis pasien hemodialisis Rumah Sakit Nur Hidayah. Dari total data, 16 pasien hemodialisis mengalami event. Mayoritas pasien hemodialisis berjenis kelamin laki-laki, yaitu 57 pasien dan perempuan 33 pasien. Kaplan-Meier dan uji log-rank menunjukkan variabel urea dan laju nadi memiliki perbedaan kurva survival antara kategori masing-masing variabel. Terdapat dua variabel yang tidak memenuhi asumsi proportional hazard sehingga model Cox yang digunakan adalah Cox time-dependent dengan fungsi waktu g(t)=t. Variabel yang berpengaruh terhadap model adalah variabel jenis kelamin dan tekanan sistolik yang diinteraksikan dengan waktu. Berdasarkan hasil diagnostic checking menggunakan Cox-Snell model Cox time-dependent menunjukkan kecenderungan kesesuaian model terhadap data.

Chronic kidney failure is a non-communicable disease that affects more than 10 percent of the world's population, including Indonesia. Chronic diseases, such as chronic kidney failure, generally cannot be cured simply by undergoing long-term treatments, such as hemodialysis, which often becomes the standard long-term care. This long medical treatment can cause anxiety or depression in patients, so in recent years complementary therapies have been developed that focus on psychological and spiritual aspects. In Indonesia, Hu-Care has been developed to support the healing process of chronic patients. Hu-Cares is expected to have a positive influence on patients, as a result helping in the healing process. This study uses Cox regression to determine the influence of psychospiritual on the survival of hemodialysis patients and carries out diagnostic checking in the Cox regression model. The data uses medical records of hemodialysis patients at Nur Hidayah hospital. From the total data, 16 hemodialysis patients experienced an event. The majority of hemodialysis patients are male, with 57 patients, and female with 33 patients. Kaplan-Meier and log-rank test indicate that urea and pulse rate variables have a survival curve difference among their respective categories. Two variables did not meet the proportional hazard assumptions, so it is determined to use the time-dependent Cox model with a time function of g(t)=t. Gender and systolic blood pressure variables interacting with time significantly influence the model. Diagnostic checking using Cox-Snell test on time-dependent Cox model confirms the model’s suitability with the data.
