Abstract

Penelitian ini diselenggarakan karena adanya kejadian potensi kecelakaan kerja jatuh dari ketinggian yang terjadi sebanyak empat kali dalam periode 2019 sampai 2022 di grup Perusahaan Orica. Berdasarkan data historis perusahaan, Kegiatan Turn Around Cooling Tower 87CT001 di PT Kaltim Nitrate Indonesia, diidentifikasi sebagai penyebab utama kecelakaan kerja. Oleh karena itu, penelitian penilaian risiko ini dilakukan untuk mengidentifikasi dan mengelola risiko kecelakaan kerja yang berpotensi merugikan karyawan dan perusahaan. Pekerjaan penggantian komponen unit, seperti Filler pack dan Drift Eliminator, di Turn Around Cooling Tower masuk dalam kategori High Potential Incident dengan potensi kecelakaan kerja, kejadian ini dapat menimbulkan cidera sampai dengan kematian. Oleh karena itu, perusahaan perlu untuk memitigasi kondisi ini. Manajemen risiko merupakan alternative pendekatan untuk menghadapi persoalan ini. Penyusunan rekomendasi merupakan hasil akhir setelah proses identifikasi, penilaian dan evaluasi risiko dilakukan. Metode yang digunakan dalam penelitian ini adalah kombinasi hazid FMEA, fishbone diagram dan PDSA. HAZID merupakan metode identifikasi risiko, selanjutnya FMEA digunakan sebagai metode penilaian risiko dan potensi bahaya. Fishbone diagram merupakan metode penentuan akar masalah dari risiko tertinggi, akar masalah menjadi acuan dalam perumusan strategi mitigasi yang akan dievaluasi menggunakan metode PDSA. Hasil evaluasi PDSA menunjukkan bahwa mengadakan refreshment training meningkatkan kesadaran pekerja Turn Around Cooling Tower.

This study was conducted since there were 4 potential falls from height that occurred four times from the period of 2019 to 2022 in the Orica Company group. Based on the historical data of this company, the Turn Around Cooling Tower 87CT001 at PT Kaltim Nitrate Indonesia was identified as the main cause of these accidents. Hence, this risk assessment study was conducted to identify and manage the risk of work accidents that could potentially harm employees and the company. Replacing unit components, such as the Filler pack and Drift Eliminator in the Turn Around Cooling Tower process is categorized as a High Potential Incident for the workers, which can cause injury, even death. Therefore, the company needs to mitigate this condition. Risk management is an alternative approach to deal with this problem. The development of recommendations is the final result after risk identification, assessment and evaluation process has been finished to be carried out. The method used in this research is the combination of Hazard Identification (HAZID), Failure Mode and Effects Analysis (FMEA), Fishbone diagram dan Plan-Do-Study-Act (PDSA). HAZID is a risk identification method, then FMEA is used as a risk assessment method and potential hazards to obtain the hazard risk level based on the Risk Priority Numbers (RPN) with the following results: (1) falling from height due to human error (RPN:80), (2) falling from height due to equipment interference (RPN:60), (3) falling from height due to worker inexperience (RPN:45). Fishbone diagram is a method of determining the main cause of the highest risk, where this cause will be used as a reference to formulate the mitigation strategies that will be evaluated by using the PDSA method. The PDSA evaluation results show that conducting refreshment training increases the awareness of Turn Around Cooling Tower workers.
