Abstract
Instalasi jacket merupakan proses akhir dalam pembuatan jacket di mana jacket ditempatkan pada lokasi yang telah ditentukan. Pada kegiatan suatu proyek risiko kecelakaan kerja selalu menjadi salah satu hal yang menjadi perhatian utama. Kecelakaan kerja ialah suatu kejadian yang dapat menyebabkan kerugian harta benda, waktu, bahkan nyawa seseorang. Maka dari itu untuk memastikan kelancaran proyek Instalasi Jacket Platform ini serta mengurangi resiko yang mungkin terjadi, diperlukan adanya sistem manajemen resiko. Pada penelitian ini digunakan metode HAZOP untuk mengidentifikasi risiko dan didapatkan sebanyak 33 risiko dalam proses Instalasi Jacket Platform. Analisis dilanjutkan dengan perhitungan tingkat risiko dominan atau kritis dan diperoleh tiga risiko dominan, dengan risiko tertinggi adalah kecelakaan saat pengelasan, konduktor terbuka, startup mesin (winch) diluar perintah. Kemudian, dilanjutkan analisis akar penyebab terjadinya risiko dominan menggunakan metode FTA dan didapatkan akar penyebab terjadinya kecelakaan kerja pada proses Instalasi Jacket Platform dengan probabilitas kejadian tertinggi adalah tekanan mekanis dan beban berlebih yang menimpa kabel serta keausan serta penuaan pada kabel dan pemasangan yang tidak sesuai standar.

Installation jacket is the final process in manufacturing jacket where jacket placed at a predetermined location. In project activities, the risk of work accidents is always one of the main concerns. A work accident is an incident that can cause loss of property, time and even a person's life. Therefore, to ensure the smooth running of the Jacket Platform Installation project and reduce risks that may occur, a risk management system is needed. In this research, the HAZOP method was used to identify risks and obtained 33 risks in the process Platform Jacket Installation. The analysis continued with calculating the dominant or critical risk level and obtained three dominant risks, with the highest risk being Accidents during welding, open conductors, machine (winch) startup outside of command. Then, we continued with the analysis of the root causes of dominant risks using the FTA method and obtained the root causes of work accidents in the process Platform Jacket Installation with the highest probability of occurrence is mechanical pressure and excessive load on the cable as well as wear and aging of the cable and installation that does not comply with standards.
