Abstract

PT INKA (Persero) adalah perusahaan yang bergerak di industri perkeretaapian yang berlokasi di Madiun, Jawa Timur. Perusahaan ini memiliki potensi risiko kecelakaan kerja yang tinggi, sehingga penerapan Sistem Manajemen Keselamatan dan Kesehatan Kerja (SMK3) menjadi hal yang sangat penting untuk menciptakan lingkungan kerja yang aman, sehat, dan produktif. Kebijakan SMK3 yang diterapkan mengacu pada peraturan nasional, seperti PP No. 50 Tahun 2012, yang mencakup penyusunan dokumen, pelaksanaan prosedur, serta pengawasan dan audit secara berkala. Penelitian ini menggunakan metode Hazard and Operability (HAZOP), yaitu suatu metode untuk menganalisis kejadian kecelakaan kerja yang pernah terjadi, dengan tujuan untuk mengidentifikasi sumber bahaya, penyimpangan, penyebab, dan memberikan tindakan perbaikan yang tercatat dalam Worksheet HAZOP. Selanjutnya, dilakukan perhitungan nilai risiko menggunakan Risk Matrix. Hasil penelitian menunjukkan adanya 6 bahaya utama dengan tingkat risiko yang bervariasi. Sebanyak 66,67% temuan tergolong dalam kategori risiko sedang, yang meliputi bahaya terkena gram, terkena mesin gerinda, terjepit hook underframe, dan terkena mesin abrasif. Sementara 33,33% termasuk dalam kategori risiko tinggi, seperti tertimpa material dan kecelakaan lalu lintas. Berdasarkan sumber bahaya, 83,33% disebabkan oleh kelalaian manusia, sedangkan 16,67% disebabkan oleh faktor kendaraan bermotor. Beberapa langkah yang diusulkan untuk mengurangi tingkat risiko tersebut antara lain adalah memberikan pelatihan kepada karyawan, mensosialisasikan kepada pekerja untuk selalu bekerja sesuai dengan SOP dan menggunakan APD yang lengkap, serta mengutamakan keselamatan dan kesehatan kerja dalam setiap aktivitas pekerjaan guna menciptakan budaya kerja yang aman dan berkelanjutan.

PT INKA (Persero) is a company operating in the railway industry, located in Madiun, East Java. The company faces a high potential risk of workplace accidents, making the implementation of the Occupational Health and Safety Management System (OHSMS) essential to creating a safe, healthy, and productive work environment. The OHSMS policy implemented adheres to national regulations, such as Government Regulation No. 50 of 2012, which covers document preparation, procedural implementation, as well as regular supervision and audits. This study utilizes the Hazard and Operability (HAZOP) method, a technique used to analyze past workplace accidents to identify sources of hazards, deviations, causes, and corrective actions recorded in the HAZOP Worksheet. Subsequently, risk values were calculated using the Risk Matrix. The study results identified six major hazards with varying risk levels. A total of 66.67% of the findings fall into the moderate-risk category, including hazards such as exposure to sparks, contact with grinding machines, being pinched by underframe hooks, and contact with abrasive machines. Meanwhile, 33.33% fall into the high-risk category, such as being struck by materials and traffic accidents. In terms of the sources of hazards, 83.33% are caused by human negligence, while 16.67% are attributed to motor vehicle factors. Several measures are proposed to reduce these risk levels, including providing employee training, educating workers to always adhere to SOPs and wear complete PPE, and prioritizing occupational health and safety in every work activity to foster a safe and sustainable work culture.
