Abstract
Proses perakitan (assembly) Accommodation Deck pada proyek Marjan GOSP-4 di PT. X memiliki tingkat risiko tinggi terhadap keselamatan dan kesehatan kerja (K3), meskipun prosedur keselamatan telah diterapkan. Penelitian ini bertujuan untuk mengidentifikasi risiko dominan, menganalisis akar penyebabnya, serta merumuskan strategi pengendalian risiko berdasarkan kerangka kerja ISO 31000. Metode yang digunakan meliputi Hazard Identification, Risk Assessment, and Risk control (HIRARC) serta Fault tree Analysis (FTA) untuk menelaah akar penyebab dari risiko ekstrem. Data dikumpulkan melalui observasi langsung dan penyebaran kuesioner kepada 14 responden lapangan. Hasil penelitian mengidentifikasi 16 risiko, dengan dua di antaranya tergolong ekstrem, yaitu cedera tulang belakang dan jatuh dari ketinggian. Akar penyebab utama meliputi kurangnya pelatihan K3, ketidakpatuhan terhadap prosedur kerja, serta kondisi lingkungan yang tidak terkendali. Mitigasi risiko direkomendasikan melalui penerapan hierarki pengendalian bahaya, peningkatan pelatihan rutin, dan evaluasi berkala terhadap Task Risk Assessment (TRA). Dengan demikian, kombinasi metode HIRARC–FTA dan pendekatan ISO 31000 terbukti efektif dalam mengidentifikasi serta merancang strategi mitigasi risiko K3 pada proyek konstruksi lepas pantai.

The assembly process of the Accommodation Deck in the Marjan GOSP-4 project at PT. X poses a high risk to occupational safety and health (OSH), despite the implementation of safety procedures. This study aims to identify the dominant risks, analyze their root causes, and formulate risk control strategies based on the ISO 31000 framework. The methods used are Hazard Identification, Risk Assessment, and Risk control (HIRARC), along with Fault tree Analysis (FTA) to examine extreme risks. Data were collected through direct observation and questionnaires distributed to 14 field respondents. The results identified 16 risks, with two categorized as extreme: spinal injury and falling from height. The main root causes include lack of OSH training, non-compliance with work procedures, and uncontrolled environmental conditions. Risk mitigation is recommended through the application of the hierarchy of hazard controls, enhancement of regular training, and periodic evaluation of the Task Risk Assessment (TRA). Therefore, the combination of HIRARC- FTA methods and the ISO 31000 approach proves effective in identifying and developing OSH risk mitigation strategies in offshore construction projects.
