Abstract

Bongkar muat petikemas merupakan salah satu aktivitas utama di pelabuhan bongkar muat yang dimana masih terdapat kecelakaan kerja, tercatat 183 insiden non-injury, 8 insiden membutuhkan penanganan medis, tanpa insiden yang menyebabkan cedera jangka panjang atau kematian, dengan lokasi insiden tertinggi terjadi di Dermaga Internasional sebanyak 15 kasus. Penelitian ini bertujuan untuk mengidentifikasi moda kegagalan tertinggi kecelakaan kerja dalam proses bongkar muat petikemas di dermaga PT. XYZ, menganalisis penyebab terjadinya moda kegagalan, serta memberikan rekomendasi pengendalian risiko yang tepat. Metode yang digunakan dalam penelitian ini adalah Failure Mode and Effect (FMEA) dan pendekatan Fuzzy-FMEA untuk meningkatkan akurasi analisis risiko. Selain itu, Fishbone Diagram digunakan untuk mengidentifikasi faktor penyebab utama kecelakaan kerja. Hasil penilaian risiko tertinggi yang dapat terjadi pada kegiatan bongkar muat petikemas di dermaga berurut yaitu mengangkat petikemas dengan kondisi twistlock terkunci dengan FRPN 7.387, beban palka tidak terangkat secara seimbang dengan FRPN 6.883 dan twistlock spreader terlepas pada pengangkatan petikemas dengan FRPN 5.999. melakukan identifikasi penyebab moda kegagalan dengan Fishbone Diagram, berikut merupakan salah satu pengelompokkan pada tipe hazard manusia adalah mengangkat petikemas dengan kondisi twistlock terkunci dengan penyebab operator tidak menyadari bahwa twistlock masih terkunci, dan twistlock mengalami kerusakan mekanis. Setelah mendapatkan risiko tertinggi dan penyebab moda kegagalan maka dilakukan pengendalian risiko, yang tepat pada moda kegagalan tersebut dengan operator crane diwajibkan untuk melakukan percobaan angkat secara perlahan sebagai prosedur awal sebelum melakukan pengangkatan dengan kecepatan penuh (hoist up full), sehingga dapat meminimalisir terjadinya kecelakaan kerja pada kegiatan bongkar muat petikemas di dermaga.

Container loading and unloading is one of the main activities at the port where there are still work accidents, 183 non-injury incidents were recorded, 8 incidents required medical treatment, with no incidents causing long-term injury or death, with the highest incident location occurring at the international jetty with 15 cases. This study aims to identify the highest failure modes of work accidents in the process of loading and unloading containers at the PT XYZ jetty, analyze the causes of failure modes, and provide recommendations for appropriate risk control. The method used in this research is Failure Mode and Effect Analysis (FMEA) and Fuzzy-FMEA approach to improve the accuracy of risk analysis. In addition, Fishbone Diagram is used to identify the main causal factors of work accidents. The results of the highest risk assessment that can occur in container loading and unloading activities at the jetty are lifting containers with the twist lock locked with FRPN 7.387, the hatch load is not lifted in a balanced manner with FRPN 6.883 and the twist lock spreader is detached in the lifting of containers with FRPN 5.999. Identifying the causes of failure modes with the Fishbone Diagram, here is one of the groupings in the human hazard type is lifting containers with the twist lock locked with the cause of the operator not realizing that the twist lock is still locked, and the twist lock has mechanical damage. After obtaining the highest risk and the cause of the failure mode, appropriate risk control is carried out on the failure mode with the crane operator required to carry out a trial lift slowly as an initial procedure before lifting at full speed (hoist up full), so as to minimize the occurrence of work accidents in container loading and unloading activities at the jetty.
