Abstract
Pembangunan jetty sebagai bagian dari infrastruktur maritim menghadirkan tantangan keselamatan kerja yang kompleks akibat kondisi lingkungan perairan, penggunaan alat berat, dan aktivitas konstruksi berisiko tinggi. Penelitian ini bertujuan untuk menganalisis risiko kecelakaan kerja pada proyek pembangunan jetty PT XYZ dengan menggunakan metode Bowtie Analysis sebagai pendekatan visual dan sistematis dalam pemetaan penyebab, kejadian, dampak, serta langkah pengendalian risiko. Identifikasi risiko dilakukan melalui penyebaran kuesioner dan penilaian menggunakan matriks risiko berdasarkan tingkat likelihood dan severity. Hasil analisis menunjukkan dua risiko dominan dalam kategori "risiko tinggi", yakni pekerja jatuh dari ketinggian saat pengecoran beton dan kegagalan struktur akibat sambungan girder yang lemah, masing-masing memiliki probabilitas intermediate sebesar 0,00350 dan 0,00784. Kedua risiko tersebut berkontribusi terhadap top event kecelakaan kerja pada proyek dengan probabilitas akumulatif sebesar 0,01131. Faktor penyebab utama mencakup kelalaian penggunaan APD, kondisi kerja tidak aman, kesalahan instalasi, serta lemahnya sistem pengawasan. Strategi pengendalian risiko disusun dalam bentuk kontrol preventif seperti pelatihan K3, inspeksi teknis, serta pengujian sambungan menggunakan metode Non-Destructive Testing (NDT), dan kontrol mitigatif seperti penempatan tim tanggap darurat, sistem peringatan dini, serta rencana kontinjensi penggantian material. Hasil penelitian menegaskan bahwa integrasi metode Bowtie dengan pendekatan kuantitatif mampu meningkatkan efektivitas manajemen risiko serta mendukung pengambilan keputusan yang lebih tepat dalam perencanaan keselamatan kerja pada proyek konstruksi maritim.

The construction of jetties, as part of maritime infrastructure development, presents complex occupational safety challenges due to the aquatic environment, the use of heavy machinery, and high-risk construction activities. This study aims to analyze occupational accident risks in the jetty construction project of PT XYZ using the Bowtie Analysis method, a visual and systematic approach for mapping causes, events, consequences, and risk control measures. Risk identification was conducted through questionnaire distribution and assessment using a risk matrix based on likelihood and severity levels. The analysis revealed two dominant risks categorized as "high risk": workers falling from heights during concrete pouring and structural failure due to weak girder connections, with intermediate event probabilities of 0.00350 and 0.00784, respectively. These risks contribute to the top event of workplace accidents in the project with a cumulative probability of 0.01131. The primary causal factors include the neglect of personal protective equipment (PPE), unsafe working conditions, installation errors, and inadequate supervisory systems. Risk control strategies were formulated through preventive measures such as occupational health and safety (OHS) training, technical inspections, and joint testing using Non-Destructive Testing (NDT) methods, as well as mitigative controls including the deployment of emergency response teams, early warning systems, and contingency plans for material replacement. The findings emphasize that integrating the Bowtie method with a quantitative approach can enhance risk management effectiveness and support more accurate decision-making in occupational safety planning for maritime construction projects.
