Abstract

Pipa bawah laut (offshore pipeline) memiliki peran vital dalam distribusi gas antarpulau, namun beroperasi dalam lingkungan yang penuh risiko. Penelitian ini bertujuan untuk menganalisis risiko kegagalan operasi pada offshore pipeline di area South Sumatera–West Java (SSWJ) dengan pendekatan Hazard and Operability Study (HAZOP) dan Bow-Tie Analysis. Identifikasi awal dilakukan melalui HAZOP untuk menemukan deviasi operasional dan potensi hazard berdasarkan parameter teknis. Penilaian risiko didasarkan pada persepsi lima ahli melalui kuesioner yang telah divalidasi dan diuji reliabilitasnya, menghasilkan nilai Likelihood Index dan Severity Index yang dikonversi ke dalam Risk Matrix. Tiga mode kegagalan dominan yang teridentifikasi, yakni korosi, fatigue, dan vandalism, kemudian dimodelkan secara mendalam menggunakan Bow-Tie Analysis. Diagram bowtie menunjukkan keterkaitan antara penyebab, kejadian utama (top event), konsekuensi, serta pengendalian preventif dan mitigatif yang relevan. Hasil penelitian ini memberikan gambaran menyeluruh mengenai peta risiko dominan serta titik kritis pengendalian, yang dapat menjadi acuan dalam strategi manajemen risiko untuk meningkatkan keselamatan dan keandalan sistem pipeline SSWJ.

Subsea pipelines play a vital role in inter-island gas distribution but operate within high-risk offshore environments. This study aims to analyze operational failure risks in the South Sumatera–West Java (SSWJ) offshore pipeline using the Hazard and Operability Study (HAZOP) and Bow-Tie Analysis methods. Initial identification was conducted through HAZOP to detect operational deviations and potential hazards based on technical parameters. Risk assessment was carried out using Likelihood and Severity scores derived from expert questionnaires, which were validated and tested for reliability, and then mapped into a Risk Matrix. Three dominant failure modes—corrosion, fatigue, and vandalism—were further analyzed using Bow-Tie diagrams to visualize the relationships among causes, top events, consequences, and the relevant preventive and mitigative controls. The findings provide a comprehensive overview of dominant risks and critical control points, serving as a practical reference for enhancing risk management strategies and improving the safety and reliability of the SSWJ pipeline system.
