Abstract
Proses inbound logistics di PT Pertamina EP Cepu Region 4 Zona 12 memiliki peran penting dalam mendukung kelancaran operasional, khususnya dalam pengelolaan supply material serta kegiatan eksplorasi dan produksi migas. Namun proses ini masih menghadapi berbagai risiko, seperti pemalsuan dokumen tender dan gangguan sistem CIVD yang disebabkan belum optimalnya penerapan sistem manajemen risiko. penelitian ini bertujuan untuk mengidentifikasi, menganalisis, dan mengevaluasi risiko pada proses inbound logistics menggunakan metode Failure Mode and Effect Analysis (FMEA) dengan menilai tingkat keparahan (Severity), frekuensi kejadian (Occurence), dan kemampuan deteksi (Detection) dari setiap potensi kegagalan untuk memperoleh nilai Risk Priority Number (RPN). Data dikumpulkan melalui observasi lapangan, dokumentasi, dan wawancara dengan expert judgement. Hasil identifikasi menghasilkan 87 risiko, terdiri atas 30 risiko pada proses procurement dan 57 risiko pada proses warehouse. Penilaian dan evaluasi menunjukkan bahwa proses procurement terdapat 70% extreme risk, 7 high risk, 20% medium risk, dan 3% low risk, sementara pada proses warehouse terdapat 33% extreme risk, 25% high risk, 39% medium risk, dan 3% low risk. Selanjutnya, dilakukan klasifikasi prioritas risiko menggunakan pendekatan Pareto yang menghasilkan 55 risiko prioritas dan 32 risiko non-prioritas. Risiko yang masuk kategori prioritas dan memiliki nilai severity minimal 9 diberikan rekomendasi penanganan sesuai hasil analisis. Selanjutnya, seluruh risiko yang telah teridentifikasi juga dipetakkan berdasarkan model Supply Chain Operations Reference (SCOR) guna untuk mengetahui proses mana yang paling banyak memiliki risiko, sehingga dapat menjadi fokus utama dalam penyusunan strategi pengendalian risiko secara terarah dan efektif.

The inbound logistics process at PT Pertamina EP Cepu Region 4 Zone 12 plays a vital role in ensuring smooth operational activities, particularly in managing material supply and supporting oil and gas exploration and production. However, this process is still exposed to various risks, such as forged tender documents and disruptions in the CIVD (Centralized Integrated Vendor Database) system, primarily duet o the suboptimal implementation of risk management systems. This study aims to identify, analyze, and evaluate risks in the inbound logistics process using the Failure Mode and Effect Analysis (FMEA) method by assessing the severity, occurrence, and detection levels of each potential failure to calculate the Risk Priority Number (RPN). Data were collected through field observations, document analysis, and interviews with expert judgement. The risk identification process resulted in 87 risks, comprising 30 risks in the procurement process and 57 risk in the warehouse process. Risk evaluation revealed that the procurement process includes 70% extreme risks, 7% high risks, 20% medium risks, and 3% low risks, while the warehouse process comprises 33% extreme risks, 25% high risks, 39% medium risks, and 3% low risks. A prioritization of these risks was then carried out using the Pareto principle, identifying 55 priority risks and 32 non-priority risks. Priority risks with a minimum severity score of 9 were further addressed through recommended mitigation strategies based on the analysis results. Additionally, all identified risks were mapped using the Supply Chain Operations Reference (SCOR) model to determine which SCOR processes are most frequently associated with risks, providing a more targeted and effective approach to risk control strategy development.
