Abstract

Pekerjaan konstruksi, khususnya pada instalasi tiang pancang jembatan di proyek reklamasi memiliki tingkat risiko kecelakaan kerja yang tinggi akibat berbagai faktor teknis dan operasional. Penelitian ini bertujuan untuk mengidentifikasi dan menganalisis potensi risiko kecelakaan kerja pada kegiatan tersebut, serta menyusun strategi pengendalian risiko yang efektif. Metode yang digunakan meliputi Failure Mode and Effect Analysis (FMEA) untuk menentukan nilai RPN, Fault Tree Analysis (FTA) untuk menelusuri akar penyebab risiko, serta bowtie analysis sebagai pemetaan visual pengendalian risiko. Hasil analisis FMEA menunjukkan bahwa risiko dominan dengan nilai RPN tertinggi adalah tiang pancang pecah dan mengenai pekerja dengan RPN 144, pekerja tertimpa material CSP dengan RPN 128, dan crane terguling karena posisi tidak stabil dengan RPN 112. Ketiga risiko tersebut dianalisis lebih lanjut menggunakan FTA dengan perhitungan minimum cut set menunjukkan bahwa risiko pekerja tertimpa material CSP yang paling mungkin terjadi dengan probabilitas 0.0036568293 dikarenanakan operator tidak melihat pekerja dengan probabilitas sebesar 0.0032678049. Pengendalian dilakukan dengan metode bowtie untuk menentukan langkah mitigasi yang tepat. Strategi pengendalian yang disarankan meliputi penerapan SOP teknis secara ketat, inspeksi alat dan material sebelum digunakan, penggunaan APD lengkap, serta pengawasan aktif oleh HSE.

Construction work, especially in the installation of bridge piles in reclamation projects has a high level of risk of work accidents due to various technical and operational factors. This study aims to identify and analyze the potential risks of work accidents in these activities, and develop effective risk control strategies. The methods used include Failure Mode and Effect Analysis (FMEA) to determine the RPN value, Fault Tree Analysis (FTA) to trace the root causes of risk, and bowtie analysis as a visual mapping of risk control. The results of the FMEA analysis show that the dominant risks with the highest RPN values are broken piles and hitting workers with RPN 144, workers hit by CSP material with RPN 128, and crane overturning due to unstable position with RPN 112. The three risks were further analyzed using FTA with a minimum cut set calculation showing that the risk of workers being hit by CSP material was most likely to occur with a probability of 0.0036568293 due to operators not seeing workers with a probability of 0.0032678049. Then control is carried out using the bowtie method to determine the right control measures. Recommended control strategies include strict implementation of technical SOPs, inspection of tools and materials before use, use of complete PPE, and active supervision by HSE.
