Abstract
Proses Loadout CO Boiler menggunakan Self-Propelled Modular Transporter (SPMT) memiliki potensi risiko kecelakaan kerja yang dapat mempengaruhi keselamatan pekerja serta kelancaran operasi.Oleh karena itu, penelitian ini bertujuan untuk mengidentifikasi dan menganalisis potensi risiko kecelakaan kerja yang mungkin terjadi selama proses Loadout CO Boiler di PT. XYZ dengan menggunakan metode Hazard and Operability Study (HAZOP) dan Fault Tree Analysis (FTA). Metode HAZOP digunakan untuk mengidentifikasi potensi bahaya dan penyimpangan operasional pada setiap tahapan proses Loadout dengan mempertimbangkan aspek teknis dan prosedural. Sementara itu, metode FTA diterapkan untuk menentukan akar penyebab dari setiap kejadian kegagalan yang telah teridentifikasi dalam analisis HAZOP. Dengan mengombinasikan kedua metode ini, penelitian ini diharapkan dapat menghasilkan pemetaan risiko yang lebih komprehensif serta memberikan rekomendasi dalam meningkatkan standar keselamatan kerja. Penelitian ini diharapkan dapat memberikan kontribusi dalam meningkatkan pemahaman terhadap risiko kecelakaan kerja dalam proses Loadout CO Boiler serta memberikan dasar bagi pengembangan strategi mitigasi yang efektif guna meningkatkan keselamatan kerja di PT. XYZ.

The Loadout process of the CO Boiler using a Self-Propelled Modular Transporter (SPMT) carries potential work accident risks that may affect worker safety and operational efficiency. Therefore, this study aims to identify and analyze potential work accident risks that may occur during the CO Boiler Loadout process at PT. XYZ using the Hazard and Operability Study (HAZOP) and Fault Tree Analysis (FTA) methods.mThe HAZOP method is used to identify potential hazards and operational deviations at each stage of the Loadout process,considering technical and procedural aspects. Meanwhile, the FTA method is applied to determine the root causes of each failure event identified in the HAZOP analysis. By combining these two methods, this study is expected to produce a more comprehensive risk mapping and provide recommendations for improving workplace safety standards.This research is expected to contribute to a better understanding of work accident risks in the CO Boiler Loadout process and serve as a basis for developing effective mitigation strategies to enhance workplace safety at PT. XYZ.
