Abstract

The CT Scan machine at Universitas Airlangga Hospital has experienced significant downtime, disrupting diagnostic efficiency and resulting in delayed patient care, reduced service quality, and increased operational costs. Despite high demand, ranging from 4,500 to 4,600 scans annually, the available CT Scan unit has not been operating at optimal capacity due to limited availability and recurring technical issues. This study aims to address these challenges through the application of the Six Sigma methodology using the DMAIC approach (Define, Measure, Analyze, Improve, and Control) to identify and eliminate the root causes of downtime. Additionally, the Failure Mode and Effect Analysis (FMEA) method was employed to assess potential failure modes and prioritize corrective actions. The analysis was based on hospital downtime records, interviews with medical staff, and historical maintenance data. Before improvements were implemented, the CT Scan machine had an availability rate of 75.385% and a sigma level of 2.853, indicating a high defect rate and low process capability. The primary causes of downtime were identified as unplanned technical failures, insufficient preventive maintenance schedules, and unfavorable environmental conditions. Based on these findings, a series of targeted countermeasures were developed, including periodic replacement of coolant, routine calibration and cleaning of the cooling system, installation of a real-time temperature monitoring system, voltage stabilizers or UPS, alarm systems for electrical fluctuations, and scheduled electrical audits. Additional recommendations included implementing a usage log and preventive replacement strategy for CT tubes, redesigning the maintenance schedule with detailed component checklists, providing technician training, and improving room ventilation through layout redesign and environmental monitoring. These improvement initiatives were validated through Monte Carlo simulation. After implementation, the study showed significant performance improvements, with reduced downtime, an increase in availability to 79.77%, and an improvement in sigma level to 3.039. This study demonstrates the effectiveness of integrating the DMAIC and FMEA methodologies in enhancing the reliability and operational performance of radiology services within a hospital setting, particularly in reducing machine downtime and improving CT Scan availability.

Mesin CT Scan di Rumah Sakit Universitas Airlangga mengalami downtime yang signifikan, sehingga mengganggu efisiensi diagnostik dan menyebabkan keterlambatan pelayanan pasien, penurunan kualitas layanan, serta peningkatan biaya operasional. Meskipun permintaan penggunaan tinggi, yaitu antara 4.500 hingga 4.600 pemeriksaan per tahun, unit CT Scan yang tersedia belum beroperasi secara optimal akibat keterbatasan ketersediaan dan masalah teknis yang berulang. Penelitian ini bertujuan untuk mengatasi permasalahan tersebut melalui penerapan metodologi Six Sigma dengan pendekatan DMAIC (Define, Measure, Analyze, Improve, dan Control) untuk mengidentifikasi dan menghilangkan akar penyebab downtime. Selain itu, metode Failure Mode and Effect Analysis (FMEA) juga digunakan untuk menilai potensi mode kegagalan dan memprioritaskan tindakan korektif. Sebelum dilakukan perbaikan, mesin CT Scan memiliki tingkat ketersediaan (availability) sebesar 75,385% dan level sigma sebesar 2,853, yang mencerminkan tingginya tingkat kegagalan dan rendahnya kapabilitas proses. Penyebab utama downtime diidentifikasi sebagai kegagalan teknis yang tidak terencana, jadwal pemeliharaan preventif yang tidak memadai, serta kondisi lingkungan yang tidak mendukung. Berdasarkan temuan tersebut, sejumlah tindakan perbaikan ditetapkan, meliputi penggantian coolant secara berkala, kalibrasi dan pembersihan sistem pendingin secara rutin, pemasangan sistem pemantauan suhu real-time, penggunaan voltage stabilizer atau UPS, sistem alarm untuk fluktuasi tegangan, serta audit listrik secara berkala. Rekomendasi tambahan mencakup pencatatan jam pakai dan strategi penggantian preventif untuk tabung CT, perancangan ulang jadwal pemeliharaan dengan checklist komponen yang rinci, pelatihan teknisi, serta peningkatan ventilasi ruang melalui desain ulang tata letak dan pemantauan kondisi lingkungan. Seluruh inisiatif perbaikan divalidasi melalui simulasi Monte Carlo. Setelah implementasi, studi menunjukkan peningkatan kinerja yang signifikan, dengan penurunan downtime, peningkatan tingkat availability menjadi 79,77%, dan peningkatan level sigma menjadi 3,039. Penelitian ini membuktikan efektivitas integrasi metodologi DMAIC dan FMEA dalam meningkatkan keandalan serta kinerja operasional layanan radiologi di lingkungan rumah sakit, khususnya dalam mengurangi downtime mesin dan meningkatkan ketersediaan unit CT Scan.
