Abstract
Kegiatan operasional penanganan kargo di terminal peti kemas memiliki peranan penting dalam mendukung kelancaran arus logistik nasional. Namun demikian, proses ini juga mengandung berbagai risiko yang dapat mengganggu keselamatan kerja, efisiensi operasional, dan kualitas pelayanan. Penelitian ini bertujuan untuk menganalisis risiko-risiko yang terdapat dalam proses operasional penanganan kargo di Terminal Peti Kemas Surabaya dengan menggunakan metode Failure Mode and Effect Analysis (FMEA). Metode ini digunakan untuk mengidentifikasi potensi kegagalan, mengevaluasi tingkat keparahan, kemungkinan terjadinya, dan kemampuan deteksi dari setiap potensi kegagalan, yang kemudian dihitung dalam bentuk Risk Priority Number (RPN). Hasil penelitian menunjukkan bahwa terdapat beberapa mode kegagalan dengan nilai RPN tinggi yang memerlukan prioritas penanganan segera, seperti pada risiko (R20.2) memiliki nilai RPN tertinggi sebesar 315, dengan risiko GPS tracking rusak yang menyebabkan rute tidak optimal dan lead time naik, (R23.8) memiliki nilai RPN sebesar 240, dengan risiko pencahayaan route yard minim dan (R9.1) dengan risiko spreader twistlock tidak terpasang dengan benar yang menyebabkan container tergelincir dan jatuh, memiliki nilai RPN sebesar 216. Semua risiko yang disebutkan merupakan very high risk. Penelitian ini merekomendasikan tindakan mitigasi berupa risk avoidance, risk mitigation, risk transfer, dan risk acceptance Dengan penerapan analisis FMEA, Terminal Peti Kemas diharapkan dapat meminimalkan risiko operasional dan meningkatkan kinerja pelayanan secara berkelanjutan.

Operational activities of cargo handling at container terminals have a strategic role in supporting the smooth flow of national logistics. However, this process also contains various risks that can disrupt work safety, operational efficiency, and service quality. This study aims to analyze the risks in the operational process of cargo handling at the Surabaya Container Terminal using the Failure Mode and Effect Analysis (FMEA) method. This method is used to identify potential failures, evaluate the severity, likelihood of occurrence, and detection capability of each potential failure, which is then calculated in the form of a Risk Priority Number (RPN). The results of the study indicate that there are several failure modes with high RPN values that require immediate priority handling, such as risk (R20.2) has the highest RPN value of 315, with the risk of damaged GPS tracking which causes the route to be suboptimal and lead time to increase, (R23.8) has an RPN value of 240, with the risk of minimal route yard lighting and (R9.1) with the risk of improperly installed spreader twistlocks which cause containers to slip and fall, has an RPN value of 216. All the risks mentioned are very high risk. This study recommends mitigation actions in the form of risk avoidance, risk mitigation, risk transfer, and risk acceptance. By implementing FMEA analysis, the Container Terminal is expected to minimize operational risks and improve service performance sustainably.
