Abstract
PT Kilang Pertamina Internasional RU IV Cilacap merupakan salah satu kilang minyak terbesar di Indonesia yang memiliki peran vital dalam penyediaan energi nasional. Untuk menjaga kelangsungan operasionalnya, diperlukan sistem manajemen risiko yang efektif, terutama dalam departemen Reliability dan Maintenance Execution. Penelitian ini bertujuan untuk mengidentifikasi dan mengevaluasi risiko kegagalan peralatan operasional berdasarkan data risk register tahun 2024 yang mendeteksi kegagalan pada 311 peralatan dari 76.000 peralatan dalam Refinery Unit IV Cilacap, serta menyusun rencana mitigasi berdasarkan hasil perhitungan dari pendekatan Fuzzy Failure Mode and Effect Analysis (Fuzzy FMEA). Pendekatan ini diterapkan untuk mengatasi subjektivitas dan ketidakpastian dalam penilaian risiko dengan memanfaatkan bilangan fuzzy terhadap parameter severity, occurrence, dan detection, melalui proses agregasi, defuzzifikasi, dan pembobotan faktor FMEA. Proses dimulai dengan identifikasi peralatan kritis, penilaian risiko, konversi nilai ke dalam fuzzy number, hingga perhitungan Fuzzy Risk Priority Number (FRPN). Hasil penelitian pada total 45 risiko yang dianalisis, hasil menunjukkan adanya pergeseran prioritas risiko dibandingkan metode konvensional, serta menghasilkan pemetaan risiko yang lebih sensitif terhadap faktor severity. Risiko dengan FRPN tinggi menjadi prioritas mitigasi dengan pendekatan avoid - reduce. Penelitian ini memberikan kontribusi dalam meningkatkan efektivitas pengelolaan risiko dan mendukung keandalan operasional di lingkungan industri migas.

PT Kilang Pertamina Internasional RU IV Cilacap is one of the largest oil refineries in Indonesia and plays a vital role in the country’s energy supply. To ensure operational continuity, an effective risk management system is essential, particularly within the Reliability and Maintenance Execution departments. This study aims to identify and evaluate the risks of equipment failure based on the 2024 risk register, which detected failures in 311 out of 76,000 pieces of equipment in Refinery Unit IV Cilacap, and to develop mitigation plans based on the results of the Fuzzy Failure Mode and Effect Analysis (Fuzzy FMEA) approach. This approach is applied to address subjectivity and uncertainty in risk assessment by utilizing fuzzy numbers for the parameters of severity, occurrence, and detection through aggregation, defuzzification, and weighting processes. The procedure involves identifying critical equipment, assessing risks, converting the values into fuzzy numbers, and calculating the Fuzzy Risk Priority Number (FRPN). Based on the analysis of 45 total risks, the results show a shift in risk priorities compared to conventional methods and produce risk mappings that are more sensitive to severity factors. Risks with high FRPN values are prioritized for mitigation using an avoid–reduce strategy. This research contributes to improving the effectiveness of risk management and supports operational reliability in the oil and gas industry.
