Abstract

Kebakaran pada ruang muat kapal tanker merupakan salah satu risiko tertinggi dalam
industri pelayaran, khususnya karena karakteristik muatan berupa bahan kimia dan bahan bakar
yang sangat mudah terbakar. Penelitian ini bertujuan untuk mengidentifikasi dan menganalisis
potensi risiko kebakaran pada ruang muat kapal tanker menggunakan metode Failure Mode and
Effect Analysis (FMEA). Pendekatan FMEA digunakan untuk mengevaluasi sistem secara
sistematis melalui penilaian terhadap tingkat keparahan (severity), kemungkinan kejadian
(likelihood) dari setiap kegagalan yang teridentifikasi. Objek penelitian adalah kapal chemical
tanker di mana tiga sistem utama yang dianalisis meliputi Nitrogen Supply System, Tank Venting
System, dan Fire Detection & Alarm System. Data dikumpulkan melalui dokumentasi teknis kapal
dan kuesioner yang disebarkan kepada kru kapal, kemudian dianalisis menggunakan matriks
risiko. Hasil penelitian menunjukkan bahwa dari sembilan peralatan pada sistem nitrogen, enam
di antaranya berada dalam kategori risiko intolerable. Pada sistem tank venting, lima dari delapan
peralatan memiliki tingkat risiko tinggi. Sementara itu, pada sistem fire detection & alarm, dua
dari empat peralatan dikategorikan dalam risiko sedang. Berdasarkan hasil tersebut, disusun
rekomendasi mitigasi yang mencakup redesign sistem, penambahan perangkat keamanan,
peningkatan pelatihan kru, dan perbaikan prosedur operasional. Penelitian ini diharapkan dapat
menjadi referensi dalam pengelolaan risiko kebakaran pada kapal tanker serta menjadi acuan
dalam pengambilan keputusan preventif dan korektif oleh perusahaan pelayaran. Dengan
menerapkan metode FMEA secara sistematis, perusahaan dapat memprioritaskan upaya
pencegahan berdasarkan data risiko aktual dan meningkatkan keselamatan operasional secara
keseluruhan.

Fire hazards in cargo holds of tanker vessels represent one of the highest risks in the
maritime industry, particularly due to the highly flammable nature of chemical and fuel cargoes.
This study aims to identify and analyze potential fire risks in the cargo hold of a tanker vessel
using the Failure Mode and Effect Analysis (FMEA) method. FMEA provides a systematic
evaluation of failure modes through assessment of severity and likelihood of occurrence of each
identified failure. The object of this research is the chemical tanker focusing on three main systems
the Nitrogen Supply System, the Tank Venting System, and the Fire Detection & Alarm System.
Data were gathered from the ship’s technical documentation and questionnaires distributed to
onboard crew, and subsequently analyzed using a risk matrix. The results show that, out of nine
components in the nitrogen system, six fall under the intolerable risk category. In the tank venting
system, five out of eight components were found to have high risk levels. Meanwhile, in the fire
detection & alarm system, two out of four components were categorized as moderate risk. Based
on these findings, mitigation recommendations were proposed, including system redesign,
enhancement of safety devices, crew training improvements, and revision of operational
procedures. This study is expected to serve as a reference for fire risk management on tanker
vessels and assist shipping companies in making preventive and corrective decisions. By applying
the FMEA method systematically, companies can prioritize mitigation efforts based on actual risk
data and improve overall operational safety.
