Abstract
Penelitian ini bertujuan untuk mengetahui pengaruh parameter ketebalan irisan, arus tabung, dan tegangan tabung CT-Scan terhadap kualitas citra dan dosis organ yang dihasilkan pada fantom antropomorfik pediatrik usia 1, 4, dan 7 tahun. Variabel kualitas citra yang ditinjau meliputi noise, Signal-to-Noise Ratio (SNR), Contrast-to-Noise Ratio (CNR), Noise Power Spectra (NPS), dan Noise Texture Deviation (NTD). Pengukuran dilakukan dengan variasi ketebalan irisan (0,625–10 mm), arus tabung (50–90 mA), serta tegangan tabung (80–120 kV). Hasil penelitian menunjukkan bahwa ketebalan irisan berpengaruh signifikan pada noise (nilai p=0,004 untuk fantom 1 tahun), SNR (nilai p≤0,001 untuk fantom 1, 4, dan 7 tahun), CNR (nilai p=0,011;0,041;0,003 untuk fantom 1, 4, dan 7 tahun), dan NPS. Arus tabung CT-Scan berpengaruh signifikan pada noise (nilai p=0,046;0,032 untuk fantom 4 dan 7 tahun) dan NPS. Tegangan tabung CT-Scan berpengaruh signifikan pada noise (nilai p=0,043;0,001;<0.001 untuk fantom 1, 4, dan 7 tahun), SNR (nilai p=0,002;0,022 untuk fantom 4 dan 7 tahun), CNR (nilai p=0,047 untuk fantom 4 tahun), NTD (nilai p=0,021;<0,001 untuk fantom 1 dan 4 tahun), dan NPS. Jika ditinjau dampaknya pada kualitas citra dan dosis organ secara umum, semakin besar ketebalan irisan maka noise akan semakin menurun dan dosis bernilai fluktuatif; semakin besar arus tabung maka noise akan semakin menurun dan dosis meningkat secara fluktuatif; dan semakin besar tegangan tabung maka noise akan semakin menurun dan dosis meningkat. Semakin besar usia fantom (secara ukuran juga semakin besar), maka noise akan semakin tinggi. Berdasarkan kualitas citra dan rata-rata dosis organ tiap protokol, kombinasi optimal menggunakan ketebalan irisan sebesar 2,5 mm, tegangan sebesar 120 kV, dan arus sebesar 70 mA (di mana ini juga cukup sesuai dengan protokol standar dosis AAPM). Hasil penelitian ini bisa menjadi acuan dalam menetapkan protokol optimal CT-Scan pediatrik berdasarkan usia pasien.

This study aimed to determine the effects of slice thickness, tube current, and CT-Scan tube voltage on image quality and organ dose in pediatric anthropomorphic phantoms aged 1, 4, and 7 years. The image quality variables reviewed include Noise, Signal-to-Noise Ratio (SNR), Contrast-to-Noise Ratio (CNR), Noise Power Spectra (NPS), and Noise Texture Deviation (NTD). Measurements were performed with variations in the slice thickness (0.625–10 mm), tube current (50–90 mA), and tube voltage (80–120 kV). The results showed that slice thickness significantly affected noise (p = 0.004 for the 1-year-old phantom), SNR (p ≤ 0.001 for the 1-, 4-, and 7-year-old phantoms), CNR (p = 0.011; 0.041; 0.003 for the 1-, 4-, and 7-year-old phantoms), and NPS. The CT-Scan tube current significantly affected the noise (p = 0.046; 0.032 for 4 and 7-year phantoms) and NPS. CT-Scan tube voltage significantly affected noise (p = 0.043, 0.001, and <0.001 for 1-, 4-, and 7-year phantoms, respectively), SNR (p = 0.002 and 0.022 for 4- and 7-year phantoms, respectively), CNR (p = 0.047 for 4-year phantoms), NTD (p = 0.021 and <0.001 for 1- and 4-year phantoms, respectively), and NPS. When viewed from its impact on image quality and organ dose in general, the greater the slice thickness, the more noise will decrease and the dose will fluctuate; the greater the tube current, the more noise will decrease and the dose will increase; and the greater the tube voltage, the more noise will decrease and the dose will increase. The older the phantom (also larger in size), the higher the noise. Based on the image quality and average organ dose for each protocol, the optimal combination used a slice thickness of 2.5 mm, voltage of 120 kV, and current of 70 mA (which also aligned well with the AAPM standard dose protocol). The results of this study can serve as a reference for determining the optimal pediatric CT scan protocol based on patient age.
