Abstract
Salah satu fitur baru yang ada pada sistem perencanaan perlakuan radioterapi atau treatment planning system (TPS) yaitu normal tissue objective (NTO) bertujuan untuk mengurangi dosis serap yang diterima oleh organ berisiko di sekitar volume target atau PTV. Penelitian ini bertujuan untuk mengetahui pengaruh apabila perencanaan menggunakan pengaturan dengan NTO dan Tanpa NTO pada kasus kanker nasofaring. Penelitian menggunakan fantom 3D printed kepala dan leher yang disinari dengan teknik IMRT dengan dosis preskripsi 6000 cGy yang terbagi atas 30 fraksi kemudian analisis verifikasi dosis dilakukan untuk mengetahui diskrepansi dosis antar dosis serap perhitungan dan TPS.Penelitian ini dilakukan untuk membandingkan bagaimana pengaruh prioritas NTO dan pengaturan fall-off NTO secara manual. Selain itu perbandingan dilakukan menggunakan indeks parameter evaluasi seperti homogeneity index (HI), conformity index (HI), dan gradient
index (GI). Variasi prioritas NTO yang ditetapkan, yaitu 50, 100, 200, dan 500. Variasi fall-off untuk Manual NTO yang ditetapkan, yakni 0,05, 0,1, 0,15, dan 0,2. Penelitian ini membuktikan bahwa nilai HI, CI, dan GI untuk Manual NTO superior daripada Otomatis NTO, meskipun dalam analisis cakupan dosis rendah pada volume organ normal, Otomatis NTO masih lebih baik daripada Manual NTO. Implementasi prioritas NTO sangat berpengaruh terhadap keberhasilan pelaksanaan terapi berdasarkan pemenuhan syarat dan rekomendasi protokol pelaksanaan radioterapi yang berlaku. Penggunaan fall-off manual sangat rumit dan cenderung tidak sekonsisten NTO Otomatis karena mengandalkan percobaan dan eror manusia
dikarenakan masih belum ada protokol yang mengatur angka masukan untuk NTO.Kata kunci: Indeks Parameter Evaluasi, Normal Tissue Objective (NTO), Sistem Perencanaan Perlakuan.

One of the new features available in modern radiotherapy treatment planning systems (TPS) is the Normal Tissue Objective (NTO), which aims to reduce the absorbed dose received by organs at risk (OARs) surrounding the target volume or Planning Target Volume (PTV). This study aims to evaluate the effects of using treatment plans with and without NTO settings in nasopharyngeal cancer cases. The research was conducted using a 3D-printed head and neck phantom irradiated with the Intensity-Modulated Radiation Therapy (IMRT) technique. A prescribed dose of 6000 cGy was delivered over 30 fractions. Dose verification analysis wasperformed to assess dose discrepancies between the calculated absorbed dose and that shown in the TPS. The study further investigates the influence of varying NTO priority and manualfall-off settings. Comparative evaluation was conducted using dosimetric indices such as the Homogeneity Index (HI), Conformity Index (CI), and Gradient Index (GI). The NTO priority values used were 50, 100, 200, and 500, while the fall-off values for manual NTO were set at 0.05, 0.1, 0.15, and 0.2. The results demonstrate that HI, CI, and GI values were superior in
manual NTO plans compared to automatic NTO, although in terms of low-dose coverage to normal tissue volumes, automatic NTO performed better than manual NTO. The implementation of NTO priority plays a significant role in achieving optimal treatment outcomes in accordance with radiotherapy protocol requirements and recommendations.However, manual fall-off configuration tends to be more complex and less consistent thanautomatic NTO, as it relies on trial-and-error by the planner due to the absence of established protocols or guidelines for specific NTO input values
