Abstract
Penilaian kualitas rencana radioterapi menggunakan teknik Three-Dimentional Conformal Radiation Therapy (3D-CRT) memerlukan parameter evaluasi yang mampu menggambarkan cakupan ukuran dosis terhadap target, keseragaman dosis, dan ketajaman penurunan dosis menuju organ sehat. Penelitian ini bertujuan mengevaluasi efektivitas indeks dosimetrik dinamis Dynamic Conformity Index (DHI), Dynamic Homogeneity Index (DHI), dan Dynamic Gradien Index (DGI) pada tiga variasi rencana 3D-CRT kanker serviks. Rencana A yakni teknik 3D-CRT tanpa optimasi, rencana B yakni 3D-CRT dengan optimasi Multi Leaf Collimator (MLC), dan rencana C yakni 3D-CRT dengan Field in Field (FiF) menggunakan fantom pelvis 3D-printed. Nilai indeks dihitung berdasarkan distribusi volume isodosis bertingkat dan diverifikasi menggunakan film dosimetri EBT4. Hasil yang didapatkan dari penelitian ini adalah nilai rata-rata DCI pada rencana A, B dan C sama di nilai 0,28. Nilai rata-rata DHI pada rencana A, B, C masing-masing secara berurutan 0,93; 0,89; 0,81. Serta nilai DGI pada masing-masing rencana secara berurutan adalah 5,16; 5,26; 4,84. Berdasarkan nilai indeks dosimetrik dinamis nilai DHI dan DGI perencanaan C lebih baik karena mendekati nilai benchmark yakni 1. Sedangkan pada DHI ditunjukkan hal yang berbeda dimana nilai DHI perencanaan A menjadi yang paling baik diantara perencanaan yang lain. Dari hasil tersebut dapat disimpulkan bahwa perencanaan C secara keseluruhan menjadi rencana paling baik jika dibandingkan dengan rencana yang lainnya. Serta indeks dosimetrik dinamis dapat menunjukkan perbedaan antar perencanaan yang lebih rinci.

The quality assessment of radiotherapy plans using the Three-Dimensional Conformal Radiation Therapy (3D-CRT) technique requires evaluation parameters that can describe the scope of the dose size to the target, dose uniformity, and sharpness of dose reduction towards healthy organs. This study aimed to determine the effectiveness of the dynamic dosimetric indices, namely, the Dynamic Conformity Index (DHI), Dynamic Homogeneity Index (DHI), and Dynamic Gradient Index (DGI), in three variations of 3D-CRT plans for cervical cancer. Plan A was a 3D-CRT technique without optimization, plan B was 3D-CRT with multi-leaf collimator (MLC) optimization, and plan C was 3D-CRT with field-in-field (FiF) using a 3D-printed pelvic phantom. The index value was calculated based on the distribution of graded isodose volumes and the truth using EBT4 dosimetry film. The results obtained from this study show that the average DCI values in plans A, B, and C were the same at 0.28. The average DHI values for plans A, B, and C were 0.93, 0.89, and 0.81, respectively. The DGI values for each plan in sequence were 5.16, 5.26, and 4.84. Based on the dynamic dosimetric index value, the DHI and DGI values of plan C were better because they were close to the benchmark value of 1. Meanwhile, the DHI shows something different, where the DHI value of plan A is the best among the other plans. From these results, it can be concluded that Plan C is the best overall plan compared with the other plans. The dynamic dosimetric index can also show differences between more detailed plans.
