Abstract

Secondary malignancy merupakan kejadian tumbuhnya tumor kedua setelah melewati perawatan terapi kanker pertama dengan potensi timbul berkisar 10% dan bergantung pada usia penderita. Secondary malignancy dianggap sebagai akibat dari tindakan radiasi atau akibat penyebaran tumor itu sendiri. Perawatan pasien secondary malignancy memiliki titik kritis pada bagian dada dan Organs At Risk (OARs) seperti Jantung, Paru-Paru, dan Sum-sum Tulang Belakang (STB) akan terkena dosis di atas rata-rata. Dosis di atas rata-rata dikhawatirkan akan menyebabkan kerusakan jaringan dan organ. Oleh karena itu dilakukan analisis metode dan dosis yang minim terhadap kerusakan OARs. Penelitian ini dilakukan menggunakan pesawat LINAC dengan energi foton 6 MV di MRCCC Siloam Semanggi Jakarta Selatan. Teknik yang digunakan dalam penelitian ini yaitu teknik 3D-CRT Field-in-Filed (FinF) dengan variasi dosis 42,4; 48; dan 50 Gy. Verifikasi dosis dilakukan dengan menggunakan film EBT3 pada fantom toraks. Hasil penelitian menunjukkan bahwa distribusi dosis dengan teknik 3D-CRT FinF mampu meminimalisir kerusakan OARs. Jantung, Paru-Paru, dan Sum-sum Tulang Belakang (STB) berada pada batas dosis yang aman untuk seluruh variasi dosis setelah dilakukan permodelan kerusakan organ. Nilai Homogenity Index (HI) dari Planning Treatment Volume (PTV) dengan metode 3D-CRT FinF 42,8 Gy, 48 Gy, dan 50 Gy secara berurutan sebesar 0,111650; 0,147517; dan 0,111807. Sedangkan nilai Conformity Index (CI) PTV dengan metode 3D-CRT FinF 42,8 Gy, 48 Gy, dan 50 Gy secara berurutan adalah adalah 0,9; 1,0; dan 1,0. Hasil pengukuran Treatment Planning System (TPS) dibandingkan dengan Film EBT3 memiliki rentang 0,060 Gy hingga 0,283 Gy dengan rentang eror 0,78 hingga 9,19 %.

A secondary malignancy is the incidence of the growth of the second primary tumor after passing the first cancer therapy treatment. The potential for secondary malignancy ranges from 10% and depends on the patient's age. A secondary malignancy is considered a result of radiation action or the spread of the tumor itself. The treatment of secondary malignancy patients has a critical point in the chest, and Organs At Risk (OARs) such as the Heart, Lungs, and spinal cord will be exposed to doses above the average. Doses above average are feared to cause tissue and organ damage. Therefore, an analysis of the method and minimal dose of OARs damage was carried out. This research was conducted using a LINAC with a photon energy of 6 MV at MRCCC Siloam Semanggi, South Jakarta. The technique used in this research is the 3D-CRT Field-in-Filed (FinF) technique with variations in doses of 42.4 Gy, 48 Gy, and 50 Gy. Dose verification was performed using EBT3 films on the thoracic phantom. The results showed that the dose distribution with the 3D-CRT FinF technique could minimize the damage to OARs. The heart, lungs, and spinal cord were at safe dose limits for all dose variations after modeling organ damage. The Homogeneity Index (HI) value of Planning Treatment Volume (PTV) using the 3D-CRT FinF method was 42.8 Gy, 48 Gy, and 50 Gy, respectively, 0.111650; 0.147517; and 0.111807. Meanwhile, the Conformity Index (CI) of PTV using the 3D-CRT FinF method of 42.8 Gy, 48 Gy, and 50 Gy, respectively, were 0.9, 1.0, and 1.0. The results of the Treatment Planning System (TPS) measurements compared with EBT3 film have a range of 0.060 Gy to 0.283 Gy with an error range of 0.78 to 9.19%.
