Abstract
Perawatan terapi radiasi memiliki tujuan untuk menghancurkan sel-sel kanker dengan memfokuskan penyinaran pada target volume agar dosis yang diterima maksimal dan meminimalkan dosis yang diterima pada organs at risk (OARs). Perawatan radioterapi pada kasus kanker payudara pasca-mastektomi kiri teknik Intensity-Modulated Radiation Therapy (IMRT) dengan menggunakan Equivalent Uniform Dose (EUD) memiliki potensi yang bagus untuk mencapai tujuan radioterapi. Penelitian ini bertujuan untuk mengevaluasi nilai out-of-field dose pada area luar Planning Target Volume (PTV), mengetahui pengaruh penggunaan EUD dan non-EUD terhadap out-of-field dose, menentukan efek radiobiologi berdasarkan nilai Normal Tissue Complication Probability (NTCP) dengan model Lyman Kutcher Burman (LKB). Perencanaan radioterapi pada Treatment Planning System (TPS) dilakukan dengan memberikan dosis total preskripsi sebesar 5000 cGy dengan variasi EUD dan non-EUD serta menggunakan teknik IMRT 6 MV 5 lapangan. Dari hasil penelitian, TPS pada area out-of-field cenderung melakukan underestimasi perhitungan dosis seiring bertambahnya jarak antara OAR ke isodose line 50%. Penggunaan parameter EUD menunjukkan distribusi dosis pada area out-of-field yang lebih sedikit dibandingkan dengan non-EUD. Perhitungan efek radibiologi berdasarkan nilai NTCP menggunakan model LKB menunjukkan nilai yang mendekati nilai ideal yakni 0% untuk keseluruhan perencanaan yang digunakan.

Radiotherapy aims to eliminate cancer cells by focusing radiation to a target volume to give a maximum dose to the target volume and minimize the dose in organs nearby. Intensity-Modulated Radiation Therapy (IMRT) radiotherapy in post-mastectomy left breast cancer using Equivalent Uniform Dose (EUD) has excellent potential to achieve radiotherapy goals. This research intends to evaluate the out-of-field dose outside Planning Target Volume (PTV) area, to find out the influence of EUD and non-EUD parameters on the out-of-field dose, and to determine the radiobiological effect by using Normal Tissue Complication Probability (NTCP) value from Lyman Kutcher Burman (LKB) model. Parameters used in this radiotherapy planning are 5000 cGy prescripted dose, EUD and non-EUD variance, and IMRT 6 MV with five fields. From the results of this research, Treatment Planning systems (TPS) in the out-of-field area tended to underestimate the dose calculation as the distance increased between the Organ at Risk (OAR) to the isodose line by 50%. Using the EUD parameter shows that the dose distribution in the out-of-field area is less exposed than the non-EUD planning. Calculation of radiobiological effects based on NTCP value using the LKB model shows that the three planning has a value close to the ideal value of NTCP, 0%.
