Abstract

Dalam menjalankan tugas dan fungsinya, RSPI Prof. Dr. Sulianti Saroso menghasilkan sejumlah besar limbah medis. Limbah medis yang tidak ditangani dengan baik akan memberikan efek negatif bagi manusia dan lingkungan. Penelitian ini bertujuan untuk mengevaluasi kondisi eksisting pengelolaan limbah medis di RSPI Prof. Dr. Sulianti Saroso serta mengkaji bagaimana kinerja insinerator dalam mengolah limbah medis. Evaluasi ini dikaji berdasarkan aspek: teknis, lingkungan, dan finansial. Pada aspek teknis dilakukan sampling selama 7 hari berturut-turut dengan total 14 sampel (2 kali sampling) serta observasi dan wawancara pada kegiatan pemilahan, penyimpanan, pengumpulan, dan pengolahan. Pada aspek lingkungan dilakukan uji lab pada emisi gas, TCLP residu abu, dan limbah cair wet scrubber insinerator sebanyak 3 kali. Sedangkan pada aspek finansial dilakukan dengan observasi dan wawancara. Evaluasi dilakukan dengan membandingkan berdasarkan PermenLHK No. P.56 Tahun 2015 dan PermenLHK No. 6 Tahun 2021. Hasil penelitian menunjukkan bahwa sistem pengelolaan limbah medis sebagian besar belum dijalan dengan benar. Jumlah timbulan dan komposisi limbah medis yaitu limbah infeksius sebesar 203 kg/hari (96,79%) dan limbah benda tajam sebesar 6,73 kg/hari (3,21%), dengan laju timbulan limbah medis 3,38 kg/pasien.hari. Emisi gas, TCLP residu abu, dan limbah cair wet scrubber menunjukkan hasil yang baik kecuali, konsentrasi Pb pada limbah cair wet scrubber yaitu 1,07 mg/L tidak memenuhi baku mutu. Biaya anggaran operasional dan maintenance insinerator selama setahun mencapai Rp. 430.383.094,-. Diperlukan perhatian lebih pada kegiatan pengelolaan limbah medis dengan meningkatkan fasilitas dan pengadaan peralatan pengelolaan yang memadai serta mengadakan pelatihan kepada cleaning service untuk meningkatkan kompetensi dan profesionalisme. Selain itu, untuk meningkatkan kinerja insinerator perlu tambahan pengujian analisis laboratorium untuk parameter POHCs dan calon dioksin (dioxin precursor) seperti, PCBs dan Total Organik Chlor.

In carrying out its duties, RSPI Prof. Dr. Sulianti Saroso generates a large amount of medical waste. Medical waste that is handled unproperly will harm humans and the environment. This study aims to evaluate the existing condition of medical waste management at RSPI Prof. Dr. Sulianti Saroso and examines the performance of incinerators in treating medical waste. This evaluation is based on three aspects: technical, environmental, and financial. In the technical aspect, sampling was conducted for seven consecutive days with 14 samples in total (2 sampling times), as well as observations and interviews on the activities such as sorting, storing, collecting, and treating medical waste. In the environmental aspect, laboratory tests were carried out on gas emissions, TCLP ash residue, and wet scrubber wastewater three times. Next, the financial aspect was conducted by observation and interviews. Evaluation is done by comparing based on PermenLHK No. P.56 of 2015 and PermenLHK No. 6 Year 2021. The results showed that the medical waste management systems had not been running properly. The amount of generation and composition of medical waste, infectious waste is 203 kg/day (96,79%), sharps waste is 6,73 kg/day (3,21%), with a medical waste generation rate of 3,38 kg/patient each day. Gas emissions, TCLP ash residue, and wet scrubber wastewater showed good results but, the Pb concentration in the wet scrubber wastewater of 1,07 mg/L did not meet quality standards. The operational and maintenance budget for the incinerator for a year reaches Rp. 430.383.094,-. More attention is needed on medical waste management activities by improving facilities, providing adequate equipment and conducting training for cleaning services to improve competence and professionalism. In addition, to improve the performance of incinerators, additional laboratory analysis tests are needed for parameters of POHCs and dioxin precursors, such as PCBs and Total Organic Chlor.
