Abstract
Mamografi dikenal secara luas dalam pencitraan klinis untuk diagnosis kanker payudara. Secara umum, payudara wanita terdiri dari jaringan adiposa dan glandular, sedangkan kanker payudara ditandai dengan adanya mikrokalsifikasi atau massa. Keberadaan kanker sulit dideteksi karena redaman sinar-X yang hampir sama dengan jaringan payudara yang sehat. Metode mamografi energi ganda dapat menekan kontras antara jaringan adiposa dan glandular sehingga meningkatkan deteksi mikrokalsifikasi atau massa. Pengurangan citra energi tinggi dan rendah adalah inti dari teknik substraksi energi ganda. Dalam studi ini, dilakukan evaluasi dosis dan kualitas citra pada teknik substraksi mamografi energi ganda menggunakan fantom in-house yang berbentuk setengah lingkaran sesuai data DICOM pasien mamografi. Fantom untuk kualitas citra terdiri dari objek serat, bintik, dan massa yang merepresentasikan kanker payudara. Lebih lanjut, penelitian ini mengevaluasi pengaruh gaya kompresi dan kombinasi target/filter terhadap dosis dan kualitas citra. Evaluasi dosis mamografi dilakukan dengan penentuan mean glandular dose (MGD) sedangkan kualitas citra dilakukan perhitungan berdasarkan parameter signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), dan modulation transfer function (MTF). Hasil menunjukkan teknik substraksi mamografi energi ganda mampu meningkatkan kualitas citra dengan nilai MGD < 3 mGy berdasarkan protokol International Atomic Energy Agency (IAEA). Pemberian gaya kompresi tidak berpengaruh terhadap dosis dan kualitas citra dari in-house fantomnya, kemudian nilai dosis dan kualitas citra dipengaruhi oleh kombinasi target/filter yang digunakan.

Mammography is widely recognized in clinical imaging for the diagnosis of breast cancer. In general, female breasts are composed of adipose and glandular tissue, whereas the presence of microcalcifications or masses characterizes breast cancer. The presence of cancer is difficult to detect because the X-ray attenuation is almost the same as that of healthy breast tissue. The dual energy mammography method can suppress the contrast between adipose and glandular tissue, thereby increasing the detection of microcalcifications or masses. The reduction of high and low-energy images is the essence of the dual energy substraction technique. In this study, the dose and image quality evaluation of the substraction technique of dual-energy mammography using a semicircular in-house phantom according to DICOM data from mammography patients was carried out. The phantom for image quality consists of fiber objects, spots, and masses that represent breast cancer. Furthermore, this study evaluates the effect of compression force and target/filter combination on dose and image quality. Evaluation of mammography dose was carried out by determining the mean glandular dose (MGD), whereas image quality was calculated based on the parameters of signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), and modulation transfer function (MTF). The results showed that the dual energy mammography substraction technique improved image quality with an MGD value < 3 mGy based on the International Atomic Energy Agency (IAEA) protocol. The compression force does not affect the dose and image quality of the in-house phantom, the dose value and image quality are affected by the target/filter combination.
