Abstract
Perencanaan perawatan radioterapi menggunakan teknik Volumetric Modulated Arc Therapy (VMAT) sering digunakan dalam pengobatan kasus kanker kepala dan leher, salah satunya pada kasus kanker orofaring. Salah satu modalitas penggunaan teknik VMAT yaitu perputaran gantry sebesar 360o menyesuaikan bentuk kontur dari kanker. Penelitian ini bertujuan untuk menganalisis nilai dosis serap pada volume target maupun OAR dari simulasi Treatment Planning System (TPS), menganalisis parameter dosimetri seperti Homogeneity index (HI) dan Conformity Index (CI) berdasarkan jumlah arc, serta memvalidasi distribusi dosis antara perencanaan dan pengukuran menggunakan film EBT3. Simulasi TPS dilakukan pada objek 3D Printed Head-Neck Phantom dengan dosis total preskripsi sebesar 6000 cGy dengan variasi jumlah arc yaitu single arc, dual arc, dan triple arc. Hasil simulasi TPS dianalisis berdasarkan rekomendasi pemberian dosis pada PTV serta batas toleransi pada OAR. Nilai dosis serap PTV pada perencanaan triple arc memenuhi optimasi dosis yang direkomendasikan ICRU Report No. 83 sedangkan dosis yang diterima OAR untuk ketiga perencanaan masih berada dalam batas toleransi beberapa referensi. Nilai HI untuk ketiga perencanaan secara berturut-turut yaitu sebesar 0,111; 0,099; dan 0,098, dimana nilai HI yang mendekati nilai ideal yaitu 0 artinya seluruh dosis pada PTV homogen. Sedangkan nilai CI untuk ketiga perencanaan secara berturut-turut yaitu 0,980; 0,989; dan 0,991, dimana nilai ideal CI yaitu mendekati 1 yang menandakan distribusi dosis tersebar rada pada Volume PTV. Hasil validasi antara pengukuran film EBT3 dengan perencanaan menghailkan perbedaan dosis yang tidak terlalu jauh untuk pembacaan dosis pada TPS dngan rentang 1cGy-10Gy berdasarkan protokol AAPM TG 218.

Radiotherapy treatment planning using Volumetric Modulated Arc Therapy (VMAT) technique is often used to treat head and neck cancer cases, one of which is oropharyngeal cancer. The VMAT technique is one of the modalities that rotate the gantry by 360o to adjust the contour of cancer. This study aims to analyze the value of the absorbed dose in the target volume and OAR from the Treatment Planning System (TPS) simulation, analysis of dosimetry parameters such as Homogeneity index (HI) and Conformity Index (CI) based on the number of arcs and validates the dose distribution between planning and measurement using EBT3 film were also investigated. TPS simulation was carried out on 3D Printed Head-Neck Phantom objects with a total prescription dose of 6000 cGy with variations in the number of arcs, namely single arc, dual arc, and triple arc. TPS simulation results were analyzed based on PTV dosing recommendations and OAR tolerance limits. The value of the absorbed dose of PTV in the triple arc design meets the recommended dose optimization of ICRU Report No. 83, while the doses received by OAR for the three plans were still within the tolerance limits of some references. The HI value for single, dual, and triple arcs was 0,111; 0,099; and 0,098. The HI value is close to the ideal value of 0, which means that all doses at PTV are homogeneous. In addition, the CI values for the three plans in a row are 0,980; 0,989; and 0,991, where the ideal value of CI is close to 1, which indicates that the dose distribution is slightly spread on the PTV volume. The validation results between EBT3 film measurements and planning resulted in a dose difference that was not too far off for dose readings at TPS with a range of 1cGy - 10Gy based on the AAPM TG 218 protocol.
