Abstract

Risiko selalu dikaitkan dengan kemungkinan terjadinya akibat buruk (mengakibatkan kerugian) yang tidak diinginkan atau datang dengan tidak terduga Pembangunan Proyek Mandiri IT Slipi merupakan proyek yang dinilai memiliki potensi risiko kecelakaan kerja yang beragam. Penelitian tugas akhir ini bertujuan untuk mengetahui risiko-risiko yang paling dominan dan mengetahui faktor-faktor penyebab dari risiko yang paling dominan tersebut serta cara pengendalian risiko tersebut. Dalam tugas akhir ini digunakan 2 metode yaitu Metode Hazard Identification, Risk Assessment, and Risk Control (HIRARC) dan Metode Domino. Metode HIRARC untuk mengetahui risiko yang paling dominan sedangkan Metode Domino digunakan untuk mengetahui dan menganalisa penyebab risiko tersebut. Proyek Pembangunan Gedung Mandiri IT Slipi dinilai memiliki risiko yang beragam. Dengan Metode HIRARC didapatkan risk assessment dari item pekerjaan dengan risiko tertinggi antara lain pekerjaan bore pile dengan risiko pekerja terperosok kedalam lubang bore, pekerjaan pengecoran dengan risiko pekerja tertimpa tremi/bucket yang lepas saat proses pengecoran, dan pekerjaan atap dengan risiko terjatuh dari ketinggian. Faktor risiko yang paling dominan, kemudian dianalisis dengan Metode Domino. Pertama terperosok dalam lubang bor masing-masing adalah lack of control (lengahnya safety control, kurang pelatihan dan kurang maksimalnya standart operational prosedure), basic couse (kurang pemahaman mengenai tempat kerja), immediate cause (tidak mematuhi himbauan dari safety control, mandor atau pelaksana) yang menimbulkan loss (Luka berat, shock, cacat, kematian (Luka berat, shock, cacat, kematian). Yang kedua tertimpa material (tremi dan bucket cor) adalah lack of control (alat kerja tidak diperiksa sebelum pekerjaan berlangsung), basic couse (sling pada tower sudah tidak memadai akibat kurangnya perawatan), immediate cause (mengangkat beban yang berlebihan) yang menimbulkan loss (Luka berat, shock, cacat, kematian (Luka berat, shock, cacat, kematian). Yang ketiga terjatuh dari ketinggian masing-masing adalah lack of control (safety management kurang memperhatikan dan memperbanyak alat pelindung diri (body herness)), basic couse (Pekerja kurang disiplin karena ingin cepat menyelesaikan pekerjaannya), immediate cause (tidak fokus dalam bekerja) yang menimbulkan loss (luka berat, cacat, kematian). Akibat yang ditimbulkan jika kecelakaan kerja terjadi adalah menurunnya produktivitas, kehilangan waktu kerja dan pekerjaan akan terhambat.

Risk is always associated with the possibility of an unwanted or unexpected adverse outcome (resulting in loss). The development of the Mandiri IT Slipi Project is a project that is considered to have various potential risks of work accidents. This final project research aims to determine the most dominant risks and determine the factors causing the most dominant risks and how to control these risks. In this final project, two methods are used, namely the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) method and the Domino method. The HIRARC method is used to determine the most dominant risk, while the Domino method is used to identify and analyze the causes of these risks. The Slipi IT Mandiri Building Project is considered to have various risks. With the HIRARC method, a risk assessment is obtained from the work items with the highest risk, including bore pile work with the risk of workers falling into the bore hole, foundry work with the risk of workers being crushed by tremors/buckets that were released during the casting process, and roofing work with the risk of falling from a height. The most dominant risk factor, then analyzed by the Domino Method. The first to fall into their respective boreholes are lack of control (lack of safety control, lack of training and less than optimal standard operational procedures), basic cause (lack of understanding of the workplace), immediate cause (not complying with the appeal of safety control, foreman or implementer)) which causes loss (severe injury, shock, disability, death). (the sling on the tower is inadequate due to lack of maintenance), immediate cause (lifting an excessive load) that causes loss (severe injury, shock, disability, death (severe injury, shock, disability, death). The third one fell from their respective heights. each is a lack of control (safety management does not pay attention to and reproduces personal protective equipment (body harness)), basic couse (workers lack discipline because they want to quickly solve problems). finish the job), immediate cause (not focused on work) which causes loss (severe injury, disability, death). The consequences if work accidents occur are decreased productivity, lost work time and work will be hampered.
