Abstract
Diabetes Mellitus Tipe-2 merupakan penyakit metabolik kronis yang ditandai dengan resistensi insulin dan peningkatan kadar glukosa darah, serta dapat menyebabkan berbagai komplikasi serius. Berdasarkan data International Diabetes Federation (IDF) tahun 2021, jumlah penderita Diabetes Mellitus Tipe-2 di Indonesia mencapai 19,5 juta jiwa dan diperkirakan meningkat menjadi 28,6 juta jiwa pada tahun 2045. Penelitian ini mengembangkan model matematika Diabetes Mellitus Tipe-2 dengan variabel kompartemen yang mencakup individu rentan tidak sadar (S_U) individu rentan sadar (S_A ) prediabetes (P), penderita diabetes tanpa komplikasi (X_W ), penderita diabetes dengan komplikasi (X_C )dan individu yang pulih (R). Model dikendalikan melalui dua variabel kontrol, yaitu edukasi melalui media (u_1 ) untuk meningkatkan kesadaran masyarakat dan pemberian terapi pengobatan (u_2 ) guna mengurangi komplikasi. Prinsip Minimum Pontryagin digunakan untuk menentukan strategi optimal dalam mengendalikan penyebaran dan komplikasi Diabetes Mellitus Tipe-2. Hasil simulasi numerik dengan metode Runge-Kutta orde 4 menunjukkan bahwa penerapan kontrol optimal mampu mengurangi jumlah individu rentan tidak sadar, menekan populasi prediabetes dan penderita diabetes dengan komplikasi, serta meningkatkan populasi yang sembuh. Kontrol optimal (u_1) dan (u_2) cenderung diterapkan secara intensif pada periode awal dan dapat dikurangi seiring menurunnya jumlah kasus. Dengan demikian, strategi kontrol yang diusulkan terbukti efektif dalam mengelola dinamika penyakit dan mengurangi beban komplikasi Diabetes Mellitus Tipe-2.

Type-2 Diabetes Mellitus is a chronic metabolic disease characterized by insulin resistance and elevated blood glucose levels, which can lead to various serious complications. According to the International Diabetes Federation (IDF) 2021 data, the number of Type-2 Diabetes Mellitus patients in Indonesia reached 19.5 million and is projected to increase to 28.6 million by 2045. This study develops a mathematical model of Type-2 Diabetes Mellitus with compartmental variables including unaware susceptible individuals (S_U), aware susceptible individuals (S_A), prediabetes (P), diabetic patients without complications (X_W), diabetic patients with complications (X_C), and recovered individuals (R). The model is controlled through two control variables: media education (u_1) to raise public awareness and treatment therapy (u_2) to reduce complications. The Pontryagin Minimum Principle is used to determine the optimal strategy for controlling the spread and complications of Type-2 Diabetes Mellitus. Numerical simulation results using the 4th-order Runge–Kutta method show that the implementation of optimal control is able to reduce the number of unaware susceptible individuals, suppress the prediabetes and diabetic populations with complications, and increase the recovered population. The optimal controls (u_1) and (u_2) tend to be applied intensively in the early period and can be reduced as the number of cases declines. Thus, the proposed control strategy proves effective in managing the disease dynamics and reducing the burden of complications in Type-2 Diabetes Mellitus.
