Abstract
PT ABC, sebuah perusahaan logistik, menghadapi tantangan signifikan dalam layanan cold chain middle mile, yang dibuktikan dengan kerugian finansial akibat kerusakan barang (basi) mencapai Rp 767.126.113 pada tahun 2024. Penelitian ini bertujuan untuk mengidentifikasi agen risiko (RA) dominan dan memprioritaskan tindakan pencegahan (PA) yang paling efektif dan rasional. Penelitian ini menggunakan pendekatan studi kasus dengan metode kuantitatif-hibrida House of Risk (HoR). Data skor Severity (S) dan Occurrence (O) diekstraksi dari data operasional historis perusahaan , sementara data korelasi (R), efektivitas (E), dan kesulitan (Dk) diperoleh dari FGD 16 responden ahli . Hasil HoR Fase 1 mengidentifikasi 13 RA, dengan RA11 (Pelanggaran Kinerja Vendor 3PL) sebagai risiko paling dominan (ARP 1215, 52% dari total risiko) , diikuti RA1 (Kelalaian SDM Internal) (ARP 186). Hasil HoR Fase 2 memprioritaskan 10 PA, di mana tiga prioritas teratas adalah PA1 (Implementasi SLA & Audit Vendor) (ETDk 2354.4), PA4 (Pengembangan Manajemen Kontingensi) (ETDk 1251.0), dan PA2 (Program Pelatihan SOP Internal) (ETDk 847.7). Penelitian ini menyimpulkan bahwa rekomendasi strategis paling rasional terfokus pada perbaikan sistem, kontrak, dan SDM, yang terbukti jauh lebih efisien daripada investasi infrastruktur aset fisik yang berbiaya sangat tinggi.

PT ABC, a logistics company, faces significant challenges in its cold chain middle-mile services, evidenced by financial losses due to spoiled goods reaching IDR 767,126,113 in 2024. This research aims to identify dominant Risk Agents (RA) and prioritize the most effective and rational Preventive Actions (PA). This study utilizes a case study approach with a hybrid quantitative House of Risk (HoR) method. Severity (S) and Occurrence (O) score data were extracted from the company's historical operational data, while correlation (R), effectiveness (E), and difficulty (Dk) data were obtained through Focus Group Discussions (FGD) with 16 expert respondents. The HoR Phase 1 results identified 13 RAs, with RA11 (3PL Vendor Performance Violations) as the most dominant risk (ARP 1215, 52% of total risk), followed by RA1 (Internal Human Resource Negligence) (ARP 186). The HoR Phase 2 results prioritized 10 PAs, where the top three priorities are PA1 (SLA Implementation & Vendor Audit) (ETDk 2354.4), PA4 (Contingency Management Development) (ETDk 1251.0), and PA2 (Internal SOP Training Program) (ETDk 847.7). This research concludes that the most rational strategic recommendations focus on system, contract, and human resource improvements, which are proven to be significantly more efficient than high-cost investments in physical asset infrastructure.
