Abstract

Single Buoy Mooring (SBM) merupakan fasilitas lepas pantai yang berperan penting dalam mendukung proses transfer minyak mentah menggunakan kapal tanker berkapasitas besar. Karakteristik operasional SBM yang berada di lingkungan laut terbuka menjadikan sistem ini rentan terhadap berbagai risiko operasional yang berpotensi menimbulkan dampak terhadap keselamatan, lingkungan, dan aset. Penelitian ini bertujuan untuk mengidentifikasi sumber bahaya, menilai tingkat risiko, serta menganalisis mekanisme kegagalan kritis pada operasional Single Buoy Mooring 150.000 DWT milik PT Kilang Pertamina Internasional. Analisis risiko dilakukan menggunakan pendekatan semi-kuantitatif melalui metode Hazard Identification, Risk Assessment, and Risk Control (HIRARC) berbasis expert judgment. Penilaian risiko dilakukan dengan menentukan nilai Likelihood of Failure (LoF) dan Consequence of Failure (CoF) menggunakan skala Likert, yang dianalisis secara terpisah pada dimensi keselamatan, lingkungan, dan aset. Tingkat risiko setiap hazard ditentukan berdasarkan nilai risiko dominan. Selanjutnya, Bow-Tie Analysis diterapkan pada hazard dengan tingkat risiko tinggi untuk memetakan hubungan antara penyebab, kejadian puncak, konsekuensi, serta pengendalian risiko. Hasil penelitian menunjukkan bahwa beberapa bahaya operasional memiliki tingkat risiko tinggi, terutama yang berkaitan dengan integritas sistem mooring dan komponen transfer fluida.

Single Buoy Mooring (SBM) is an offshore facility that plays a critical role in supporting crude oil transfer operations using large-capacity tankers. The operational characteristics of SBM systems in open-sea environments expose them to various operational risks that may affect safety, the environment, and asset integrity. This study aims to identify operational hazards, assess risk levels, and analyze critical failure mechanisms of a 150,000 DWT Single Buoy Mooring operated by PT Kilang Pertamina Internasional. Risk assessment was conducted using a semi-quantitative approach based on the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) method supported by expert judgment. Likelihood of Failure (LoF) and Consequence of Failure (CoF) were evaluated using a Likert scale and analyzed separately for safety, environmental, and asset dimensions. The dominant risk for each hazard was determined by selecting the highest risk value among these dimensions. Subsequently, Bow-Tie Analysis was applied to high-risk hazard to map causal factors, hazard, consequences, and existing risk control measures. The results indicate that several operational hazards fall into high-risk categories, particularly those related to mooring system integrity and fluid transfer components.
