Abstract
The concept of social security have been known to be existed in order to provide a full protection on individuals against the loss of resources, such as health problems. However, its acknowledged that even Indonesia does not have the social security which adequately fulfill the users need, especially for people with Down Syndrome whose disability last a life-time. Based on this situation, a thorough exploration toward its condition, level severity, the recoverability alternatives, its required cost, and recoverability which they can achieve need to be conducted to evaluate the current operationalization of social security program in Indonesia, which is BPJS. In order to do that, a series of data collection method such as interview, survey, even literature review have been done to get the whole picture on how the BPJS operationalization system work in giving coverage for people with down syndrome. The result itself were made into a Causal Loop Diagram as the system thinking approach. This method combined with financial modelling resulting on the total cost which the family need to prepare for at least the 21 years of the down syndrome life. It shows that the higher severity level need higher recovery cost with the respective number to be -Rp741,118,543.41 for mild, -Rp820,875,028.02 for moderate, and -Rp906,757,506.66 for severe and profound. This calculation also means that the government is in higher risk due to unable to provide a higher coverage which can improve the recoverability of the down syndrome and because the increase in the number of down syndrome cases in the future. The findings of this research suggest that it is very difficult to raise a down syndrome children without the help of insurance. But more importantly, the zero program on down syndrome and inadequate coverage shown the urgency of social security design that can improve inequality.
