Abstract
Kebutuhan minyak bumi pada kehidupan sehari-hari semakin meningkat. Minyak berasal dari dasar laut, sehingga perlu adanya fasilitas yang memadai saat proses eksplorasi dan eksploitasi minyak mentah. Proses penyaluran minyak mentah ke FSO Arco Ardjuna PT PHE ONWJ dibantu dengan pipa product transfer system. Pada proses tersebut terdapat kemungkinan kegagalan yang terjadi dari beberapa faktor yaitu dari alat, cuaca dan kondisilaut, maupun manusia. Pada penelitian ini, dibahas mengenai analisis risiko kegagalan operasi saat loading-unloading minyak mentah. Analisis dilakukan menggunakan metode HAZOP Analysis dan Bow-Tie Analysis, dengan HAZOP Analysis untuk identifikasi bahaya dan Bow- Tie Analysis untuk menemukan mitigasi yang sesuai. Dari metode tersebut maka didapatkan gambaran mengenai risiko-risiko yang dapat terjadi sehingga bisa dilaksanakannya pengendalian risiko yang dibutuhkan oleh FSO Arco Ardjuna. Dari hasil penelitian inididapatkan bahwa kegagalan operasi saat loading-unloading crude oil FSO Arco Ardjuna PT PHE ONWJ didapatkan sebanyak 41 potensi bahaya, dengan risiko tertinggi adalah pada inadequate quality of the transfer equipment components dengan likelihood rank adalah 4 dan severity rank adalah 4. Hasil visualisasi Bow-Tie dari risiko dominan didapatkan 5 penyebab yaitu korosi, kualitas material kurang memadai, material terkelupas, umur pemakaian, dan degradasi material dan 5 konsekuensi yaitu properti rusak, kegiatan operasional tertunda, tumpahan minyak, kerusakan lingkungan akibat tumpahan minyak, dan terjadi korosi pada peralatan transfer produk.

The need for petroleum in daily life is increasing. The oil comes from the seabed, so it is necessary to have adequate facilities during the exploration and exploitation of crude oil. The process of distributing crude oil to FSO is assisted by the Product Transfer System pipeline. In this process there is a possibility of failure that occurs form several factos, namely tools, weather and sea conditions, and human. In this study, the analysis of the risk of operational failure during loading-unloading of crude oil is discussed. The analysis was carried out using the HAZOP Analysis and Bow-Tie Analysis methods. HAZOP Analysis for hazard identification, and Bow-Tie Analysis for finding the appropriate mitigation. From this methods, an overview of the risks than can occur can be obtained so that the risk control required by FSO Arco Ardjuna can be implemented. From the results of this study, it was found that there are 41 potential hazards on the process of loading-unloading crude oil FSO Arco Ardjuna PT PHE ONWJ, with the highest risk is inadequate quality of the transfer equipment components with the likelihood rank is 4 and severity rank is 4. The results of Bow-Tie visualization of dominant risk found five causes, namely corrosion, inadequate material quality, eroded material, service life, and material degradation, and five consequences, namely property damage, delayed operational activities, oil spills, environmental damage due to oil spill, and corrosion occurs on the product transfer equipment.
