Abstract

Kanker merupakan penyakit paling mematikan di dunia. Kanker disebabkan oleh pertumbuhan sel abnormal yang tidak terkendali dalam tubuh yang dapat merusak sel normal lain bagian tubuh. Salah satu kanker yang paling banyak diderita adalah kanker payudara. Tingkat keparahan kanker payudara dibagi menjadi 5 stadium, dari stadium 0 sampai 4. Untuk kanker payudara stadium IV, memiliki tingkat keparahannya tersendiri dilihat berdasasrkan letak metastasis terbentuk. Urutan keparahan kanker payudara stadium IV dari yang paling berbahaya samai yang tidak adalah multiple metastasis, metastasis otak, metastasis hati, metastasis paru-paru, dan metastasis tulang. Sampai saat ini belum diketahui secara pasti faktor-faktor yang berpengaruh terhadap keparahan kanker payudara stadium IV. Oleh karena itu penelitian perlu dilakukan untuk mengetahui faktor apa saja yang berpengaruh terhadap keparahan kanker payudara stadium IV. Penelitian ini menggunakan metode regresi logistik ordinal dengan unit penelitian pasien penderita kanker payudara stadium IV di RS Kanker Dharmais Tahun 2021. Berasarkan hasil penelitian, didapatkan bahwa pasien penderita kanker payudara stadium IV di RS Kanker Dharmais yang terbanyak adalah pasien yang mengidap metastasis tulang sebanyak 78 pasien atau 40% dari 197 pasien. Dari hasil analisis didapatkan empat model logit dengan faktor yang berpengaruh terhadap keparahan kanker payudara stadium IV. Faktor yang berpengaruh adalah lama menderita dan status reseptor progresteron. Diketahui pula nilai odds ratio variabel lama menderita sebesar 1,047 dan variabel status reseptor progresteron sebesar 2,499.

Cancer is the deadliest disease in the world. Cancer is caused by the uncontrolled growth of abnormal cells in the body that can damage normal cells in other parts of the body. One of the most common cancers is breast cancer. The severity of breast cancer is divided into 5 stages, from stage 0 to 4. For stage IV breast cancer, it has its own severity level based on where the metastases are formed. The order of severity of stage IV breast cancer from the most dangerous to the least is multiple metastases, brain metastases, liver metastases, lung metastases, and bone metastases. Until now, it is not known with certainty the factors that influence the severity of stage IV breast cancer. Therefore, research needs to be done to find out what factors affect the severity of stage IV breast cancer. This study uses the ordinal logistic regression method with the research unit being patients with stage IV breast cancer at the Dharmais Cancer Hospital in 2021. Based on the results of the study, it was found that the most patients with stage IV breast cancer at the Dharmais Cancer Hospital were patients with bone metastases, as many as 78 patients or 40% of 197 patients. From the analysis results obtained four logit models with factors that affect the severity of stage IV breast cancer. The influencing factors are the duration of the illness and the status of the progesterone receptor. It is also known that the odds ratio for the long-suffering variable is 1.047 and the progesterone receptor status variable is 2.499.
