Abstract
Adolescent Idiopathic Scoliosis adalah salah satu skoliosis yang menyerang remaja dan umumnya perempuan. Seiring pertumbuhan tulang mereka, diperlukan assessment untuk melihat perkembangan skoliosisnya yang dilakukan per tahunnya. Proses assessment ini umumnya dilakukan menggunakan x-ray dan pengukuran dilakukan menggunakan Cobb Angle. Radiasi yang dihasilkan x-ray bisa memberikan efek negatif pada pasien terutama pada perempuan. Alternatif lain diperlukan untuk proses assessment ini. Belakangan, alternatif lain yang diunggulkan yaitu menggunakan ultrasound dan Spinous Process Angle atau SPA sebagai parameter pengukuran. Pada penelitian ini citra diambil secara mandiri kepada 7 subjek berbeda. Kemudian, citra melalui dua proses yaitu proses pre-processing dan pengukuran. Pada proses pre-processing, citra diperbaiki kualitas citranya dengan menggunakan CLAHE, Median Filter. Lalu, citra direkonstruksi dan dilakukan proses RGB to Gray dan invert image. Kemudian, hasil citra yang telah direkonstruksi dicari kurva yang terbentuk dengan mencari intensitas maksimum pada setiap baris di citra tersebut. Alamat piksel dari baris tersebut kemudian dicatat dan dibuat regresi polinomial. Hasil regresi polinomial kemudian dicari posisi paling kiri atau paling kanan untuk mencari kurva skoliosis. Pada kurva kemudian diambil 3 titik pada kurva untuk diambil menjadi poin pengukuran dari Spinous Process Angle. Hasil otomatis ini kemudian dibandingkan dengan metode manual yang diambil dengan mencari kurva dengan yang didapat dari gradient vector flow. Hasil pengukuran kemudian dievaluasi menggunakan Pearson’s Correlation dengan membandingkan SPA yang telah didapatkan dengan Cobb Angle. Hasil Pearson’s Correlation untuk SPA manual yang didapatkan sebesar 0.2 dan SPA otomatis sebesar 0.25 yang menunjukkan Spinous Process Angle dan Cobb Angle memiliki korelasi positif yang rendah. Kedua hasil ini dibandingkan dan mendapat Pearson’s Correlation sebesar -0.15. Hasil ini menunjukkan bahwa korelasi antara metode otomatis dan manual berbanding terbalik. Sistem pengukuran otomatis pada citra ultrasound bisa menjadi alternatif untuk mengukur angle scoliosis dengan beberapa penyesuaian agar bisa dipakai di kemudian hari.

Adolescent Idiopathic Scoliosis is a scoliosis that affects adolescents and generally women. As their bones grow, an assessment is needed to see the progress of their scoliosis which is done annually. This assessment process is generally carried out using x-rays and measurements are carried out using Cobb Angle. The radiation produced by x-rays can have a negative effect on patients, especially women. Another alternative is needed for this assessment process. Recently, another alternative that has been favored is using ultrasound and Spinous Process Angle or SPA as measurement parameters. In this study, images were taken independently of 7 different subjects. Then, the image goes through two processes, namely the pre-processing and measurement processes. In the pre-processing process, the image quality is improved by using CLAHE, Median Filter. Then, the image is reconstructed and the RGB to Gray and invert image processes are carried out. Then, the results of the reconstructed image are searched for the curve formed by finding the maximum intensity in each line in the image. The pixel address of the row is then recorded and a polynomial regression is made. The results of the polynomial regression are then searched for the leftmost or rightmost positions to find the scoliosis curve. On the curve then 3 points are taken on the curve to be taken as measurement points from the Spinous Process Angle. This automatic result is then compared with the manual method taken by finding the curve with that obtained from the gradient vector flow. The measurement results were then evaluated using Pearson’s Correlation by comparing the obtained SPA with Cobb Angle. The Pearson’s Correlation result for manual SPA is 0.2 and 0.25 for automatic SPA, which indicates that the Spinous Process Angle and Cobb Angle have a low positive correlation. These two results were compared and got a Pearson’s Correlation of -0.15. These results indicate that the correlation between automatic and manual methods is inversely proportional. The automatic measurement system on ultrasound images can be an alternative to measuring the angle of scoliosis with some adjustments so that it can be used in the future.
