Abstract

Keandalan operasional lokomotif diesel elektrik sangat dipengaruhi oleh efektivitas sistem pemeliharaan berkala dalam mencegah terjadinya kegagalan komponen yang dapat mengganggu keselamatan dan kelancaran perjalanan kereta api. Laporan magang ini bertujuan menganalisis pelaksanaan pemeliharaan preventif serta mengidentifikasi potensi kegagalan mekanik pada lokomotif CC 201 dan CC 203 di UPT Depo Lokomotif Kelas A Sidotopo, PT Kereta Api Indonesia (Persero) Daop 8 Surabaya. Pengumpulan data dilakukan melalui observasi lapangan, wawancara teknis, dan studi dokumentasi terhadap riwayat kerusakan lokomotif selama periode Februari hingga Maret. Analisis permasalahan dilakukan menggunakan metode 5W+1H untuk mengidentifikasi penyebab, lokasi, serta tindakan perbaikan, sedangkan metode Failure Mode and Effect Analysis (FMEA) digunakan untuk menentukan tingkat prioritas risiko berdasarkan nilai Risk Priority Number (RPN) yang diperoleh dari parameter Severity, Occurrence, dan Detection. Hasil analisis menunjukkan delapan jenis kerusakan dominan pada rangka bawah lokomotif, yaitu keretakan skin plate, hilangnya corong pemasir, keausan carbon brush motor traksi, penyusutan diameter roda dan flens, penipisan blok rem, ketidaksesuaian jarak toraks silinder rem, kekurangan pelumas pada axle cap dan gearbox, serta penutup axle cap yang kotor. Berdasarkan hasil FMEA, komponen roda, sistem pengereman, dan motor traksi merupakan komponen dengan tingkat prioritas penanganan tertinggi. Rekomendasi yang diusulkan meliputi peningkatan kualitas inspeksi harian, penguatan manajemen suku cadang, serta pelaksanaan kalibrasi dan pemeliharaan berkala secara konsisten. Penerapan metode 5W+1H dan FMEA secara terintegrasi terbukti mampu mendukung pengambilan keputusan teknis dalam kegiatan pemeliharaan sehingga berkontribusi terhadap peningkatan keandalan armada dan umur pakai komponen lokomotif.

The operational reliability of diesel-electric locomotives is strongly influenced by the effectiveness of preventive maintenance in minimizing component failures that may compromise railway safety and service continuity. This industrial internship report aims to analyze the implementation of periodic maintenance and identify potential mechanical failures in CC 201 and CC 203 locomotives at UPT Depo Lokomotif Kelas A Sidotopo, PT Kereta Api Indonesia (Persero) Regional Operation 8 Surabaya. Data were collected through field observations, technical interviews, and documentation studies of locomotive maintenance records from February to March. Problem analysis was conducted using the 5W+1H method to identify the causes, locations, and corrective actions for each failure, while Failure Mode and Effect Analysis (FMEA) was employed to prioritize risks based on the Risk Priority Number (RPN), calculated from the Severity, Occurrence, and Detection ratings. The results identified eight major failure modes in the locomotive underframe, including cracked skin plates, missing sanding funnels, worn traction motor carbon brushes, reduced wheel tread and flange dimensions, worn brake blocks, improper brake cylinder torque distance, insufficient lubrication in the axle cap and gearbox, and contaminated axle cap covers. The FMEA results indicate that the wheel assembly, braking system, and traction motor are the highest-priority components requiring corrective action. Recommended improvements include strengthening daily inspection practices, enhancing spare-part management, and implementing regular calibration and preventive maintenance programs. The integration of the 5W+1H and FMEA methods proved effective in supporting technical decision-making, extending component service life, and improving the long-term reliability of the locomotive fleet.
