Abstract

Sistem Informasi Laboratorium (LIS) merupakan komponen kritis ekosistem kesehatan digital yang mengelola data pasien sensitif. Implementasi Undang-Undang Perlindungan Data Pribadi No. 27/2022 dan Peraturan Menteri Kesehatan No. 24/2022 menuntut standar keamanan tinggi, namun di lapangan muncul kesenjangan antara regulasi dan praktik, termasuk pengiriman hasil laboratorium melalui kanal informal seperti WhatsApp demi kecepatan layanan. Penelitian ini menganalisis faktor-faktor yang memengaruhi niat kepatuhan privasi (Compliance Intention) pengguna LIS menggunakan model Sequential Dual-Calculus yang mengintegrasikan Protection Motivation Theory (PMT) dan Privacy Calculus Theory (PCT). Penelitian menggunakan desain concurrent embedded mixed-methods dengan komponen kuantitatif (PLS-SEM dan IPMA) sebagai metode primer dan komponen kualitatif (analisis tematik ATLAS.ti) sebagai metode embedded. Data dikumpulkan dari 310 Ahli Teknologi Laboratorium Medik (ATLM) anggota DPC PATELKI Kota Semarang melalui kuesioner daring berisi 31 item Likert dan 3 pertanyaan terbuka. Analisis kuantitatif dilakukan dengan SmartPLS 4 (bootstrap 5.000 subsampel, two-tailed), sedangkan 773 quotation kualitatif dianalisis menggunakan ATLAS.ti 23. Hasil pengujian menunjukkan seluruh sembilan hipotesis diterima. Empat kontribusi teoretis diidentifikasi: (1) Reverse Privacy Paradox, persepsi kerentanan berpengaruh negatif terhadap motivasi perlindungan (β = −0,078; T = 1,964; p = 0,050), didukung 72 quotation (23,2%) ATLM yang menyatakan tidak ada hambatan; (2) Mandatoriness Paradox, biaya kepatuhan berpengaruh positif terhadap motivasi (β = +0,154; p = 0,004), dengan rasio keluhan prosedur vs tuntutan SOP/audit = 2:59; (3) Validasi Sequential Dual-Calculus, Protection Motivation berpengaruh kuat terhadap Privacy Concern (β = 0,455; f² = 0,284); dan (4) Privacy Concern sebagai determinan utama Compliance Intention dengan efek yang besar (β = 0,693; f² = 0,925). IPMA mengidentifikasi Privacy Concern, Protection Motivation, dan Privacy Benefit sebagai tiga konstruk prioritas. Implikasi praktis mengarahkan pada strategi peningkatan kesadaran privasi, pelatihan berbasis kompetensi, dan tata kelola penegakan kepatuhan untuk meningkatkan kepatuhan privasi pengguna LIS di Indonesia.

Laboratory Information Systems (LIS) are critical components of digital healthcare ecosystems managing sensitive patient data. Indonesia's Personal Data Protection Law No. 27/2022 and Ministry of Health Regulation No. 24/2022 demand high security standards; however, gaps persist between regulation and practice, including informal transmission of laboratory results via WhatsApp for service expediency. This study analyzes factors influencing privacy Compliance Intention among LIS users using a Sequential Dual-Calculus model integrating Protection Motivation Theory (PMT) and Privacy Calculus Theory (PCT). A concurrent embedded mixed-methods design was employed with PLS-SEM and IPMA as the primary quantitative method and ATLAS.ti thematic analysis as the embedded qualitative component. Data were collected from 310 Medical Laboratory Technologists affiliated with DPC PATELKI Semarang through an online questionnaire containing 31 Likert-scale items and 3 open-ended questions. Quantitative analysis used SmartPLS 4 (bootstrap 5,000 subsamples, two-tailed), while 773 qualitative quotations were analyzed using ATLAS.ti 23. All nine hypotheses were supported. Four theoretical contributions were identified: (1) Reverse Privacy Paradox, where Threat Vulnerability negatively influences Protection Motivation (β = −0.078; T = 1.964; p = 0.050), supported by 72 quotations (23.2%) stating no barriers exist; (2) Mandatoriness Paradox, where Response Cost positively influences motivation (β = +0.154; p = 0.004), with a complaint-to-demand ratio of 2:59; (3) Sequential Dual-Calculus Validation, where Protection Motivation strongly predicts Privacy Concern (β = 0.455; f² = 0.284); and (4) Privacy Concern as the primary determinant of Compliance Intention with a large effect (β = 0.693; f² = 0.925). IPMA identified Privacy Concern, Protection Motivation, and Privacy Benefit as priority constructs. Practical implications direct strategies toward privacy awareness enhancement, competency-based training, and compliance governance to improve LIS user privacy compliance in Indonesia.
