Abstract

Resistensi insulin pada otot rangka secara fundamental berakar dari kegagalan transduksi sinyal proksimal antara protein Insulin Receptor Substrate-1 (IRS-1) dan Phosphoinositide 3-kinase (PI3K). Penelitian ini bertujuan untuk membandingkan dinamika ekspresi IRS-1 dan PI3K pada sel otot normal versus hiperinsulinemia, serta membuktikan apakah kerusakan pada jalur IRS-1/PI3K tersebut menjadi penyebab utama kegagalan translokasi GLUT4. Penelitian in-vitro ini menggunakan galur sel otot T48-CTF yang dipapar pada kondisi lingkungan normal dan hiperinsulinemia (insulin 50 nM). Untuk memvalidasi mekanisme kerusakan sinyal, eksperimen penyelamatan (rescue experiment) menggunakan kombinasi lima blocker (JNK, MAPK, mTORC, AS160, dan PKCζ) turut diaplikasikan. Pengamatan dilakukan secara time-series (menit ke-10 hingga 50) dengan tingkat ekspresi protein dikuantifikasi menggunakan metode Enzyme-Linked Immunosorbent Assay (ELISA) pada panjang gelombang 450 nm. Hasil penelitian menunjukkan bahwa pada lingkungan normal, ekspresi IRS-1 dan PI3K berjalan seirama dan stabil, yang memicu translokasi GLUT4 secara optimal (mencapai 6,539 ng/mL pada menit ke-30). Sebaliknya, hiperinsulinemia memicu desensitisasi berupa lonjakan awal IRS-1 (21,360 ng/mL pada menit ke-10), yang kemudian anjlok drastis akibat hiperfosforilasi serin dan degradasi proteosomal (7,703 ng/mL pada menit ke-50). Degradasi IRS-1 ini secara struktural menyebabkan PI3K kehilangan situs pengikatan, sehingga mengalami penurunan menjadi 5,403 ng/mL pada menit ke-40. Terputusnya poros kritis IRS-1/PI3K ini menyebabkan berhentinya instruksi ke hilir, yang terbukti melumpuhkan eksositosis vesikel GLUT4 (menyusut hingga 1,555 ng/mL). Berdasarkan hal tersebut, dapat disimpulkan bahwa kerusakan fisik dan fungsional pada interaksi jalur sinyal IRS-1 ke PI3K terbukti secara saintifik sebagai penyebab utama kegagalan translokasi GLUT4 yang mengunci sel otot dalam status resistensi insulin. ================================================================================================================================
Insulin resistance in skeletal muscle fundamentally stems from proximal signal transduction failure between Insulin Receptor Substrate-1 (IRS-1) and Phosphoinositide 3-kinase (PI3K). This study aimed to compare the expression dynamics of IRS-1 and PI3K in normal versus hyperinsulinemic muscle cells, and to prove whether the disruption of the IRS-1/PI3K pathway is the primary cause of GLUT4 translocation failure. This in-vitro study utilized the T48-CTF muscle cell line exposed to normal and hyperinsulinemia (50 nM insulin) environments. To validate the signal disruption mechanism, a rescue experiment using a combination of five blockers (JNK, MAPK, mTORC, AS160, and PKCζ) was applied. Observations were conducted in a time-series (minutes 10 to 50), with protein expression levels quantified using an Enzyme-Linked Immunosorbent Assay (ELISA) at 450 nm. The results showed that in a normal environment, IRS-1 and PI3K expressions were synchronized and stable, triggering optimal GLUT4 translocation (reaching 6.539 ng/mL at minute 30). Conversely, hyperinsulinemia triggered desensitization characterized by an initial IRS-1 spike (21.360 ng/mL at minute 10), which drastically plummeted due to serine hyperphosphorylation and proteasomal degradation (7.703 ng/mL at minute 50). This IRS-1 degradation structurally deprived PI3K of its docking site, causing its concentration to drop sharply to 5.403 ng/mL at minute 40. The severance of this critical IRS-1/PI3K axis halted downstream instructions, which was proven to paralyze GLUT4 vesicle exocytosis (shrinking to 1.555 ng/mL). It is concluded that the physical and functional damage to the IRS-1 to PI3K signaling pathway is scientifically proven as the primary cause of GLUT4 translocation failure, locking muscle cells in an insulin-resistant state.
