Abstract

Perkembangan teknologi mendorong transformasi digital di berbagai sektor, termasuk kesehatan. Kementerian Kesehatan Republik Indonesia mendukung hal ini melalui Cetak Biru Transformasi Digital Kesehatan 2024 dan Peraturan Menteri Kesehatan Republik Indonesia Nomor 24 Tahun 2022 tentang Rekam Medis, yang mewajibkan fasilitas layanan kesehatan menggunakan rekam medis elektronik (RME). Namun, masih terdapat layanan kesehatan yang belum menerapkan RME sesuai ketentuan, berisiko menyebabkan duplikasi data hingga kesulitan pencarian data. Klinik Pratama Menganti Gresik, yang telah menerapkan RME dengan format SOAP (Subjective, Objective, Assessment, Plan), masih memiliki keterbatasan dari sisi pengalaman pengguna bagi tenaga kesehatan sebagai end-user. Penelitian ini mengembangkan aplikasi web RME dengan studi kasus klinik tersebut, menerapkan metode ICONIX Process sebagai SDLC serta pendekatan SOAP untuk meningkatkan pengalaman pengguna dan kualitas pencatatan yang lebih terstruktur. Sistem dibangun secara full-stack menggunakan Next.js dengan PostgreSQL sebagai RDBMS dan Prisma sebagai ORM, serta mendukung standar FHIR R4 untuk interoperabilitas dengan SATUSEHAT. Hasil penelitian menunjukkan seluruh 13 use case berhasil diimplementasikan, dengan blackbox testing menunjukkan 211 dari 211 test case berhasil, heuristic evaluation menindaklanjuti 26 dari 27 temuan usability, serta UAT memperoleh skor rata-rata 4.00 dari skala 5 dan skor SUS sebesar 74.17 dari benchmark 68. Dengan demikian, sistem RME yang dikembangkan berhasil membantu operasional Klinik Pratama Menganti Gresik sesuai dengan kebijakan Kementerian Kesehatan Republik Indonesia.

Technological development has driven digital transformation across various sectors, including healthcare. The Indonesian Ministry of Health supports this through the 2024 Health Digital Transformation Blueprint and Ministerial Regulation No. 24 of 2022 on Medical Records, which requires healthcare facilities to adopt electronic medical record (EMR) systems. However, some healthcare facilities in Indonesia have yet to implement EMR as required, risking data duplication and difficulty in data retrieval. Klinik Pratama Menganti Gresik, which has implemented an EMR system with SOAP (Subjective, Objective, Assessment, Plan) documentation, still faces limitations in user experience for healthcare workers as end-users. This research develops a web-based EMR application using the clinic as a case study, applying the ICONIX Process method and the SOAP approach to improve user experience and ensure more structured, high-quality documentation. The system is built full-stack using Next.js with PostgreSQL as the RDBMS and Prisma as the ORM and supports the FHIR R4 standard for interoperability with SATUSEHAT. The results show that all 13 use cases were successfully implemented, with blackbox testing showing 211 out of 211 test cases passing, heuristic evaluation addressing 26 out of 27 usability findings, and UAT achieving an average score of 4.00 out of 5, along with a SUS score of 74.17 against a benchmark of 68. Accordingly, the developed EMR system has successfully supported the operations of Klinik Pratama Menganti Gresik in accordance with the policies of the Indonesian Ministry of Health.
