Abstract
Transformasi digital kesehatan di Indonesia mewajibkan fasilitas pelayanan kesehatan mengimplementasikan Rekam Medis Elektronik (RME) yang terintegrasi dengan Platform SATUSEHAT menggunakan standar Fast Healthcare Interoperability Resources (FHIR). Namun, tingkat integrasi yang masih rendah menunjukkan bahwa pengembangan RME belum sepenuhnya berorientasi pada kebutuhan pengguna. Penelitian ini bertujuan mengusulkan redesign workflow RME pada fasilitas kesehatan tingkat pertama (FKTP) dengan mengintegrasikan Design Thinking, Agile Development, dan pemanfaatan Large Language Models (LLM) sebagai co-pilot pada tahap ideasi dan prototyping. Penelitian dilaksanakan di Puskesmas Sumobito, Kabupaten Jombang, melalui tahapan Empathize, Define, Ideate, Prototype, dan Test. LLM dimanfaatkan menggunakan pendekatan role-based prompting, few-shot prompting, dan chain-of-thought prompting untuk membantu mengeksplorasi kebutuhan pengguna, menghasilkan alternatif solusi workflow, dan menyusun rekomendasi rancangan antarmuka yang tetap mempertimbangkan interoperabilitas HL7 FHIR dan SATUSEHAT. Evaluasi prototype melibatkan 12 tenaga kesehatan yang terdiri dari 5 petugas admisi, 6 perawat/bidan, dan 1 dokter menggunakan Maze Usability Testing, System Usability Scale (SUS), dan open-ended questions. Hasil penelitian menghasilkan redesign workflow pada proses admisi, anamnesis dan tanda-tanda vital, serta diagnosis dan clinical impression yang mampu menyederhanakan alur pelayanan dan mengurangi dokumentasi berulang. Pemanfaatan LLM mendukung eksplorasi alternatif solusi yang selanjutnya divalidasi bersama Subject Matter Experts (SME). Hasil pengujian menunjukkan prototype memiliki tingkat usability yang baik dengan nilai SUS sebesar 79,25 (kategori Above Average – Good).

Digital health transformation in Indonesia mandates healthcare facilities to implement Electronic Medical Records (EMR) integrated with the SATUSEHAT Platform using the Fast Healthcare Interoperability Resources (FHIR) standard. However, the low level of integration indicates that EMR development is not yet fully oriented toward user needs. This study aims to propose a redesign of the EMR workflow at primary healthcare facilities (FKTP) by integrating Design Thinking, Agile Development, and the utilization of Large Language Models (LLM) as a co-pilot during the ideation and prototyping stages. The research was conducted at the Sumobito Public Health Center (Puskesmas), Jombang Regency, through the stages of Empathize, Define, Ideate, Prototype, and Test. LLMs were utilized using role-based prompting, few-shot prompting, and chain-of-thought prompting approaches to assist in exploring user needs, generating alternative workflow solutions, and formulating interface design recommendations while considering HL7 FHIR and SATUSEHAT interoperability. The prototype evaluation involved 12 healthcare professionals, consisting of 5 admission staff, 6 nurses/midwives, and 1 doctor, using Maze Usability Testing, the System Usability Scale (SUS), and open-ended questions. The results of the study yielded a redesigned workflow for the admission, anamnesis and vital signs, as well as diagnosis and clinical impression processes, which successfully simplified the service flow and reduced redundant documentation. The utilization of LLMs supported the exploration of alternative solutions, which were subsequently validated with Subject Matter Experts (SMEs). The testing results showed that the prototype possessed a good level of usability, achieving a SUS score of 79.25 (Above Average – Good category).
