Abstract

Indonesia is the world's largest producer of palm oil, and Central Kalimantan is one of the leading production regions, home to PT Musirawas Citraharpindo, which operates Palm Oil Mill (PKS) MC 1 in Seruyan Regency. The company does not yet have a documented and structured risk analysis system across its Fresh Fruit Bunch (FFB) to Crude Palm Oil (CPO) production chain, meaning potential process failures cannot be identified, prioritized, and addressed systematically. To address this problem, this research applies the Failure Mode and Effects Analysis (FMEA) method across the entire FFB-to-CPO production chain at PKS MC 1. Risk identification was carried out in two stages, beginning with a literature review that produced 28 initial failure modes, followed by structured field interviews with the Production Supervisor and Plant Manager that added 19 company-specific failure modes, bringing the total to 47 failure modes across all eight processing stations. Each failure mode was rated using the Severity, Occurrence, and Detection parameters on a scale of 1 to 10, and the Risk Priority Number (RPN) was calculated to determine the priority level of each risk, producing a combined RPN total of 8,048. A total of 11 failure modes were classified as Critical Risk, 28 as Major Risk, and 8 as Moderate Risk, with none falling into the Minor Risk category. By applying the Pareto principle, 32 failure modes were selected as priority risks, together accounting for approximately 80% of the total RPN, and one additional failure mode with a Severity rating of 10 was included regardless of its Pareto ranking, bringing the total to 33 prioritized failure modes. A Risk Avoidance strategy, involving structural and engineering controls such as mechanical interlocks and mandatory verification gates, was recommended for 12 failure modes classified as Critical Risk or rated Severity 9-10, while a Risk Mitigation strategy, targeting improvements in Occurrence and Detection through strengthened SOPs and training, was recommended for 21 failure modes classified as Major or Moderate risk. This research provides PT Musirawas Citraharpindo with a structured, prioritized, and evidence-based risk management methodology to support CPO quality assurance, occupational safety, and continuous ISPO compliance at PKS MC 1.
