Abstract

PT Rajawali Tanjungsari Enjiniring memiliki proses produksi karung plastik yang terdiri atas jalur woven bag dan inner bag yang saling berkaitan. Data tahun 2025 menunjukkan 3.191 catatan gangguan yang didominasi shuttle, benang/rak, dan plastic link/line. Pada periode yang sama, terdapat 419.276 unit barang sortir/reject, dengan BS RAK dan BS SHUTTLE mencapai 60,97% dari total. Pola tersebut mengindikasikan konsentrasi permasalahan pada proses weaving/circular loom, meskipun tingkat risikonya tetap perlu dinilai berdasarkan dampak, kemungkinan kejadian, dan kemampuan deteksi. Penelitian ini menggunakan AS/NZS 4360, IDEF0, FMEA, analisis Pareto, risk matrix, dan DEMATEL. Hasil penelitian mengidentifikasi 109 failure mode valid; 63 di antaranya membentuk 80,17% kontribusi kumulatif RPN. Risk matrix menghasilkan 28 risiko Extreme, 59 High, 14 Medium, dan delapan Low. Sebanyak 15 failure mode dianalisis dengan DEMATEL, terdiri atas tujuh faktor cause dan delapan faktor effect. Empat core factor pada setup circular loom, setup sealing, penyelarasan inner, dan kondisi area seal dipantau lebih intensif karena memiliki pengaruh dan keterlibatan tinggi. Risk treatment diprioritaskan melalui sistem peringatan stok bobbin, master setting dan pembatas posisi setup, pengendalian parameter sealing, sensor dan kamera inspeksi, serta pengujian seal dan penahanan produk tidak sesuai untuk mencegah perambatan kegagalan ke proses berikutnya.

PT Rajawali Tanjungsari Enjiniring operates an interconnected plastic sack production system consisting of woven bag and inner bag lines. In 2025, 3,191 production disturbance records were identified, dominated by shuttle, yarn/rack, and plastic link/line issues. During the same period, 419,276 sorted/rejected units were recorded, with BS RAK and BS SHUTTLE accounting for 60.97% of the total. These patterns indicate a concentration of problems in the weaving/circular loom process, although the risk level must still be assessed based on impact, likelihood, and detectability. This study applies AS/NZS 4360, IDEF0, FMEA, Pareto analysis, a risk matrix, and DEMATEL. The results identified 109 valid failure modes, of which 63 contributed 80.17% of the cumulative RPN. The risk matrix classified 28 risks as Extreme, 59 as High, 14 as Medium, and eight as Low. Fifteen failure modes were analyzed using DEMATEL, comprising seven cause factors and eight effect factors. Four core factors related to circular loom setup, sealing setup, inner alignment, and seal-area conditions require more intensive monitoring because of their high influence and involvement. Risk treatment is prioritized through bobbin-stock warning systems, master settings and setup-position guides, sealing-parameter control, sensors and inspection cameras, seal testing, and containment of nonconforming products to prevent failure propagation to subsequent processes.
